If so, HealthRIGHT 360 is currently looking to fill a Nurse Complex Care Manager position opening at our Tenderloin Health Services in San Francisco! The ideal candidate has knowledge of contemporary case management principles, has the ability to work professionally with clients suffering from mental illness, substance abuse and homelessness, and is able to work within a frequently changing project scope while maintaining overall direction and structured priorities. In this position you will gain a deep understanding of the drivers responsible for poor health outcomes in a safety net clinic, become familiar with complex challenges, as well as work alongside an enthusiastic team of medical and behavioral health providers. This position will be based at our Tenderloin Health Services clinic which is located in the historic Glide building in the Tenderloin neighborhood of San Francisco.  It is a vibrant, fast-paced, dynamic clinic that seeks to provide care in non-traditional models and is determined to meet clients “where they are at” by employing harm-reduction methods and continuous judgment-free unconditional care.

Key Responsibilities

  • Must have critical thinking ability to effectively plan and provide patient care consistent with standards and regulations and deal with unique problems as they arise
  • Maintain current knowledge of community resources and develop working relationships with those resources for clients
  • Provide medication education, management, and monitor patient adherence
  • Participate in developing comprehensive care plan with and for patients and coordinate care as outlined in care plan
  • Provide regular follow-up and maintain contact with clients as needed to assure completion of care plan, and provide regular feedback to primary provider of care and staff regarding clients’ progress

Education and Knowledge, Skills and Abilities

  • Education: Bachelor’s Degree in Nursing from an accredited college or university, MSN preferred OR Master’s Degree in Social Work from a CSWE accredited university. PharmD, DPT, MSOT or other degree associated with clinical license may be considered.
  • Experience: Three (3) years of progressively responsible and directly related work experience
  • License/Certification: CA Registered Nurse (RN) or CA Licensed Clinical Social Worker (LCSW). Other clinical licenses and degrees may be considered (PT, OTR, RT, Pharm). Accredited Case Manager (ACM) preferred
