Health to Hope Clinics (Urban Community Action Projects)

Also known as Urban Community Action Projects, Health to Hope Clinics’ health care services affirm the value of delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate and culturally effective. Health to Hope believes that low-income and homeless individuals have the right to health and social services that, in addition to treatment and care, focus on empowerment and enhancing each individual’s capacity to understand and manage his or her health needs.

At Health to Hope, patients receive care from experienced and caring providers. These providers are focused on treating immediate health issues while addressing potential future health concerns through a model of care that integrates primary care, behavioral health care, dental care and case management services.
