Harlem United is a community health center that strives to be the premier multiservice organization dedicated to eradicating HIV and related health disparities among the most vulnerable populations (i.e. homeless and HIV positive individuals) in the Upper Manhattan neighborhoods.

We offer a broad scope of services, such as, Primary Care, Behavioral Health, Dental, Day Treatment, Prevention and Housing. We have a central Harlem location at 123-125 West 124th Street and a location in Spanish Harlem at 179 East 116th Street.

Services

To increase access and eliminate barriers to care, the HCH Program will operate during daytime, evening and weekend hours, and will employ culturally and linguistically appropriate staff. Most importantly, the HCH Program will operate a comprehensive coordinated, high-level quality medical care program within a system that fully engages the targeted individuals into medical care, while also integrating clients into a wide realm of supportive services.

The HCH Program will operate out of two sites: 123-125 West 124th Street and 179 East 116th Street. The scope of services will be responsive to the specific needs of the homeless population, and will include preventive health services, management of chronic conditions, mental health, substance use, and dental services, referrals, outreach utilizing a Lay Health Promoter approach, and intensive case management. 24 hour emergency on-call, Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Electronic Medical Records, Flu Vaccinations, Gynecology, Health/Nutritional Education, Hepatitis C screening/treatment, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mental Health Services for Youth, Mobile Van/Transportation, Multidisciplinary Health Teams, Outreach, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Residential, Services in Supportive Housing, Smoking Cessation, Specialty Care, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Urgent Care

Harlem United program operates daytime, weekend and evening hours and employs culturally sensitive bi-lingual staff in our two locations: 123-125 West 124th Street and 179 East 116th Street. We offer comprehensive high level coordinated quality care between medical, dental, susbstance abuse and behavioral health divisions.

All HCH clients receive a dedicated case manager who coordinates their health care services and can assist with any housing and medical insurance needs. We have linkages throughout the City with ten facilities for specialty medical care, seven agencies, including two within our parent division Harlem United, for harm reduction and needle exchange and 51 facilities for behavioral health and substance abuse inpatient and outpatient treatment. We also offer an Adult Day Care Program for qualified HIV positive clients.
