Southwest Community Health Center – McKinney Health Care for the Homeless

Southwest Community Health Center (SWCHC) is a nonprofit organization providing comprehensive health care for medically under served individuals in Bridgeport, Connecticut since 1977.  The McKinney Health Care for the Homeless program, which serves the Greater Bridgeport area, has been funded since 1987 and provides a full range of services to those people experiencing homelessness. In 2013 over 100,000 patients were seen at Southwest. Close to 1,500 visits occurred at shelter and transitional housing locations.

Services

The goal of the McKinney Health Care for the Homeless Program (McKinney HCH) is to provide comprehensive health care services to men, women and children in the greater Bridgeport area who are without a home. This includes those people who are staying in a shelter, transitional housing, and living on the street or in a car. Southwest realizes that many individuals and families lose their housing due to loss of a job, house fires, death of the family’s provider and many other reasons. Those people may have no other option than to stay temporarily in the home of a friend or relative until they can obtain their own permanent housing.
The Southwest McKinney HCH program provides services to these individuals and families, particularly those with high-risk pregnancies and chronic diseases which require constant monitoring and treatment. The Mckinney HCH Program provides adult and pediatric primary medical care and specialty care services including: WIC, behavioral health, HIV/AIDS, podiatry, dental care, laboratory services, obstetrics & gynecology, and nutrition.
The Homeless Program is based at SWCHC’s Marina Village location, 743 South Avenue, Bridgeport, CT. All patients are also welcome to receive services at any of our locations. The McKinney HCH Program provides primary medical care in licensed clinic rooms in several Bridgeport area shelters, faith-based substance abuse shelters, and transitional housing programs. The McKinney HCH staff also collaborates with other organizations who provide shelter to the homeless such as: Bridgeport Rescue Mission, Refocus Ministries, Alpha Community Services/YMCA, Goodwill and the Homeless Outreach Treatment Team (HOTT). The McKinney HCH team consists of: the RN coordinator, 2 APRN’s, the LPN case manager, and 2 LPN’s who work as a team with the APRN’s.
