The Henrietta Johnson Medical Center (HJMC) Homeless Health Care Access Program expands access to primary health care for persons who are homeless in Wilmington.
24 hour emergency on-call, Asthma, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Gynecology, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Obstetrics, Outreach, Pediatrics, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Well Child Check-ups, Podiatry Services
HJMC maintains a specialist referral network located throughout the Wilmington area to serve all patients requiring additional medical and dental treatment. Physicians at HJMC have staff and admitting privileges to all hospitals in the area. Working relationships have been established with mental health and substance abuse programs, social service organizations, community centers, churches, and civic organizations. The project coordinator at HJMC works with Ministry of Caring case managers to refer individuals in need of such services to the appropriate programs.
