Public Health Management Corporation (PHMC)

Member of the Council badgePhiladelphia’s Health Care for the Homeless Program, administered by Public Health Management Corporation, provides primary health care, social services and behavioral health care in Philadelphia, and through sub-contract in Delaware County. Outreach is provided at 40 shelters, day programs and other facilities that serve the homeless. Comprehensive physical health, behavioral health, family planning and social services are provided at the Mary Howard Health Center, located in center city Philadelphia. Additional HCH sites where primary care is delivered are the Stenton Family Manor and People’s Emergency Shelter, both in family shelters. Under HCH are two primary care in public housing sites: The Rising Sun Health Center and the PHMC Health Connection. Within the last year HCH added the Care Clinic, a primary care site for people with HIV/AIDS. Effective December 2011, HCH has added an additional health center, Congreso de Latinos Unidos (Congreso). The center serves the medically under and uninsured, homeless, chronically ill, low-income populations.

Services

HCH provides family planning, behavioral health and social services at shelter-based clinics as well as at HCH health centers and public housing sites. The project is part of the Governor's Diabetes Collaborative. Optometric services and nutritional services are also provided. Dental is NOT provided on-site. 24 hour emergency on-call, Adult/Vocational Education, Basic Needs, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Child Development Services, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Housing placement, Immunizations, Interpretation services, Labs, Medication Prescribing, Medication Dispensing, Mental Health Counseling, OB/GYN, Optometry Services, Outreach, Pediatrics, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, Services in permanent supportive housing, Shelter outreach, Specialty Care, STD Testing, Street Outreach, Substance Abuse Treatment, TB screening/education, Tobacco cessation, Well Child Check-ups

Linkages

The project has linkages to all major City departments, substance abuse programs, HIV/AIDS organizations, the Family Planning Council, hospitals, domestic abuse programs and homeless and housing advocacy programs. Staff have knowledge of (and often linkage to) transitional and permanent housing programs, food cupboards and other services.
