People’s Community Health Clinic (PCHC) has a homeless health care team which provides outreach services and refers persons who are homeless to needed services.
The homeless health care team visits shelters, street locations, and transitional housing units to assess persons who are homeless, to encourage them and arrange for them to receive medical, substance abuse, and mental health services. PCHC's physicians, PAs, and nurse practitioners provide preventive and acute health care services. A certified substance abuse counselor is on staff for evaluations of clients. The center also has a dental clinic. 24 hour emergency on-call, Asthma treatment, Basic Needs, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Child Development Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Documenting Disability Services, Domestic Violence services, Electronic Medical Records, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Interpretation services, Labs, Medication Prescribing, Mental Health Counseling, Outreach, Pediatrics, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, STD Testing, Street Outreach, TB screening/education, Tobacco cessation, Urgent Care, Well Child Check-ups, X-rays
People's Clinic is a member of the local Homeless Coordinating Board. PCHC works cooperatively with local antipoverty agencies, the food bank, shelters, County relief offices, VA, and other State and Federal agencies providing services to persons who are homeless. Linkages are also in place with faith based organizations who assist individuals who are homeless with their basic needs. PCHC has a referral relationship with Black Hawk/Grundy Mental Health Center for mental health services. Pathways and Covenant Medical Center are local organizations that provide chemical dependency treatment.
