Optimus Health Care, Inc. is a federal qualified health center (FQHC) as of the year 1976. The Center serves lower Fairfield County in medically under served areas including the City of Bridgeport, City of Stratford, and the City of Stamford, Connecticut. Optimus Health Care, Inc. is comprised of eleven primary health care delivery sites, six school based health centers, it manages an SBHC in the City of Stratford, three homeless health center sites, and a Women Infant and Children (WIC) program – in collaboration with Southwest Community Health Center. Optimus Health Care, Inc.’s goals and mission are to provide primary health care and supplemental services to the uninsured and under-insured. The HCH program operated by Stamford Community Health Center offers health education, oral health, mental health, and substance abuse services. These services are offered at the Shelter for the Homeless and at health center sites.

Linkages include the U.S. Department of Housing and Urban Development (HUD) Greenwich/Stamford Continuum of Care, Shelter for the Homeless, F.S. Dubois Center, Viewpoint Recovery Program, St. Luke’s LifeWorks, Stamford Hospital, and the City of Stamford Health Department.
