Neighborhood Health Association – Mildred Bayer Clinic

Neighborhood Health Association (NHA) administers the Senior Service Center and operates 5 community health centers, one of which is a womans clinic and one is Mildred Bayer Clinic for the Homeless and a pediatric center. Mildred Bayer delivers primary medical, dental and vision care. Services provided on site or within the NHA network include podiatry, pediatric care, OB/GYN, social services, WIC, transportation, health education, case management, outreach, pharmacy and nurses that visit 5 different shelters.


Asthma treatment, Basic Needs, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Child Development Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Interpretation services, Labs, Medication Prescribing, Medication Dispensing, Mental Health Counseling, Mobile Medical Services, OB/GYN, Optometry Services, Outreach, Pediatrics, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, Shelter outreach, Specialty Care, STD Testing, Street Outreach, TB screening/education, Tobacco cessation, Transportation Services, Well Child Check-ups, X-rays


Linkages include shelters, transitional facilities, chemical dependency programs, mental health facilities, Toledo Area Alliance to End Homelessness and Greater Toledo Continuum of Care.
