Neighborhood Health Association – Mildred Bayer Clinic

Since 1984, the Mildred Bayer Clinic has provided clinical services for the homeless. Becoming part of NHA helped the clinic expand services. With its 2016 move into a newer and larger facility, services were expanded to include dental, case management and vision. Those who use the Mildred Bayer Clinic also have access to all other services offered by NHA, including but not limited to pharmaceuticals, transportation, cardiology, general surgery, audiology, WIC, pediatrics, women’s health, obstetrics and gynecology.

Eligibility For Services
• Living On The Street
• Staying In A Shelter or Transitional Housing
• Doubling Up (staying with a friend or family member)

Pharmacy Access
Any patient seen at Mildred, whether they have insurance including Medicare or not, who is below 200% of the poverty is eligible for a medication voucher.
Must Have a Prescription from Mildred Bayer Clinic for The Homeless Provider
Provides 30 Days’ Worth of Medications with No Refills

Additionally, we have Patient Navigators on staff. Patient Navigators help you fill out paperwork to determine eligibility for service as well as receive referrals for additional treatment or transportation. The clinic also provides personal hygiene kits, a change of clothes, private restrooms, and shower facilities.

At Neighborhood Health Association, we strongly believe that choosing to manage one’s healthcare is choosing to actively control one’s life. It is our hope that our services nurture the spirit that empowers people to take such control.

All services are available to all NHA patients regardless of where they receive their care. No one is turned away from care, regardless of their ability to pay.

Linkages

Linkages include shelters, transitional facilities, chemical dependency programs, mental health facilities, Toledo Area Alliance to End Homelessness and Greater Toledo Continuum of Care.
