Metro Community Provider Network Homeless Project
Services include primary care medical services, substance abuse and mental health counseling, case management, outreach, human service referrals, and other forms of assistance. Each clinic is equipped with multiple exam rooms and is staffed by a physician, nurse practitioner, and case management/care coordinators/outreach services. All clients have access to other services in the larger MCPN system, including colposcopy, ultrasound, physician support and consultation. Through referral arrangements, clients have access to mental health, substance abuse, dental and pharmaceutical services. MCPN has numerous linkages in the community to serve individuals who are homeless. Through referral mechanisms and on-site access, services are provided by Aurora Mental Health, Jefferson Center for Mental Health, Aurora Housing Authority, Comitis Crisis Center, Inter-Faith Ministries, Tri-County Health Department, Jefferson County Health Department, and the Colorado Coalition for the Homeless.
