Mendocino Community Health Clinic (MCHC) has been providing comprehensive primary health services in rural northern California since 1992. Achieving FQHC designation in 1994, MCHC began its homeless program in 2004. MCHC is the largest safety net provider for ambulatory care in Mendocino and Lake Counties with community health centers located in Ukiah, Lakeport and Willits.

MCHC has linkages with the Ukiah Community Center, Ford Street Project, Buddy Eller Shelter, Plow Shares Community Dining Room, Mendocino County AIDS/Viral Hepatitis Network, the Ukiah Valley Medical Center (hospital), and the Mendocino County Sheriff's Office. MCHC participates in the Homeless Services Planning Group as well as the Chronic User's System of Care.
