Ingham County Health Department

Sparrow Community Health Center is one of nine stand-alone centers within the community health center network of the Ingham County Health Department. It is the designated site for the provision of health care services to homeless individuals. Outreach and service delivery is provided at homeless shelters by a mobile health team.

Primary care medical services comprise the bulk of services rendered. Augmenting these services are participation in Title X (Family Planning), Title XV (Breast and Cervical Cancer Control Program), Vaccines for Children, on-site substance abuse treatment, and smoking cessation counseling. STI/HIV screening and treatment is available as well as enabling services by a social worker, nutritionist, and other support staff. Other services, such as dental, are provided through referral within Ingham County’s Community Health Center Network.

Networking with a local neighborhood center has yielded improved access to health care. Involvement with Michigan State University (MSU) Extension service offers a referral source to assist with chronic disease management. Attending physicians from MSU provide volunteer after hours medical services and limited access to specialty care on-site.
