Healthcare for the Homeless – Houston
Healthcare for the Homeless – Houston (HHH) provides care in shelters, transitional living programs, soup kitchens, and on the streets. A consortium of 31 social service agencies, educational institutions, homeless/formerly homeless consumers, public agencies, and health care providers form the HHH Advisory Council.
Services
Services, provided at three shelter clinics and through street outreach, include primary care, outreach, women and children's clinic, dentistry, podiatry, vision care, pharmacy, TB/STD testing, immunizations, mental health/substance abuse counseling, case management, health education, referrals and jail inreach project. Asthma, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Gynecology, Hepatitis C screening/treatment, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mobile Van/Transportation, Multidisciplinary Health Teams, Optometry Services, Outreach, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Urgent Care
