Harbor Care Clinic is a new access point grantee made possible through funding from the ARRA. On June 30, 2009, Harbor Care Clinic began providing primary and preventive care services to people experiencing homelessness in the greater Nashua area. Harbor Care Clinic operates primarily from one fixed location with two fully equipped exam rooms. Outreach and health education are also conducted at shelters and other areas frequented by individuals who are homeless.
Services include primary medical care, medication, substance abuse couseling and referrals, mental health counseling and referrals, laboratory and diagnostic testing, medical/social care coordination, and eligibility screening. Harbor Care Clinic utilizes electronic medical record and practice management systems. 24 hour emergency on-call, Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Gynecology, Health/Nutritional Education, Hepatitis C screening/treatment, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Multidisciplinary Health Teams, Outreach, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Substance Abuse Treatment, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness
Harbor Care Clinic has established linkages with health and social service agencies throughout the greater Nashua area including Lamprey Health Care, a 330(e) grantee also located in Nashua.
