Santa Barbara County Public Health Department – Health Care for the Homeless
The HCH Project provides health and support services to people experiencing homelessness throughout county. Access to health care is provided through coordinated outreach integrated with an elaborate and effective clinic system.
The system of care has three satellite clinics located within homeless shelters, five Public Health Department Health Care Centers, five non-profit sub-recipient community clinics and one contracted dental center that collaborate to serve all areas of the County. The field service team consists of Public Health Nurses (PHNs) stationed in thirteen homeless shelters and transitional living centers. The nurses provide triage care and make referrals to the Health Care Centers and shelter-based clinics for those needing additional care.
Coalitions, Casa Esperanza and Good Samaritan shelters, Santa Barbara Rescue Mission, Salvation Army, Transition House, Bridge House, and Hope Center.
