Community Health Centers, Inc., operates Healing Hands Health Care, which provides primary health care, case management and substance abuse services for Oklahoma County. Healing Hands provides direct transportation from homeless shelters to our clinic. Our main clinic is in downtown Oklahoma City with outreach clinics at YWCA, Jordan’s Crossing, WestTown Day Center, and the Salvation Army. Healing Hands provides linkages to housing, specialty care, WIC, mental health services and dental services.

Services

The program operates an ambulatory health care facility in downtown Oklahoma City that provides on-site primary care access to individuals across the lifespan. The clinic is accessible to the community’s three shelters and the downtown bus station. Clinic staff includes physicians, nurses, a mental health counselor, case manager, program assistant, and program coordinator.

A wide range of primary health care services is provided on-site, including medical care, preventive health services, well child care, immunizations, HIV education/testing, counseling, family planning, pregnancy testing, mental health counseling, adult immunizations, WIC, and pharmaceuticals. Limited diagnostic laboratory testing is available. Case management activities include transportation arrangements, eligibility assistance for Medicaid, housing resource identification, crisis intervention, and remedial education. Women and Children Health Access Program (WCHAP), funded by Oklahoma City’s Continuum of Care, offers primary health care to women and children at three shelter clinics and on-site at HHHCS.

Services include 24 hour Emergency On-Call, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Contraceptives (issued w/counseling), Diabetes Testing/Care, Electronic Medical Records, Family Planning, Health/Nutritional Education, HIV/AIDS Screening Education, Immunizations, Interpretation Services, Labs, Medication Prescribing, Medication Dispensing, Mobile Medical Services, OB/GYN, Outreach, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, Shelter Outreach, SOAR Services, STD Testing, Street Outreach, Substance Abuse Treatment, TB Screening/Education, Tobacco Cessation, Transportation Services, Urgent Care, Well Child Check-Ups. WCHAP staff includes a family nurse practitioner, licensed practical nurse, and case manager.

Linkages

Linkages are in place for dental care and complex laboratory studies. HHHCS maintains referral linkages with St. Anthony Hospital and University Hospitals for emergency medical, radiology services, specialty medical services, and OB/GYN care. HHHCS has established linkages with day centers, emergency shelters, mental health/substance abuse counseling, and other social service agencies.
