Care Alliance Health Center
Founded in 1985, Care Alliance Health Center (CAHC) is a non-profit, Federally Qualified Health Center (FQHC) focusing on the unique health care needs of the homeless, individuals living in public housing, and the underserved. From humble beginnings in a church basement, CAHC has grown to four free standing sites, a mobile clinic, and numerous outreach locations. CACH is recognized as a Level III Patient Centered Medical Home (PCMH) that delivers team-based, culturally competent primary care to adults and children. Our unique Homeless Outreach Program visits shelters, drop-in centers, and campsites to engage the homeless. CAHC was an early adopter of physical-behavioral health integration (PBHI) and now offers Medication-Assisted treatment for individuals struggling with alcohol and opioid addiction.
Services include primary care, dental care, HIV testing and tracking, short-term mental health and chemical dependency treatment and referral, and referrals for specialty care through community linkages. Through outreach services and primary care clinics in public housing and other locations, Care Alliance is able to provide a seamless system of service delivery across the life span continuum. 24 hour emergency on-call, Asthma treatment, Basic Needs, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Documenting Disability Services, Electronic Medical Records, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Housing placement, Immunizations, Interpretation services, Labs, Medication Prescribing, Mental Health Counseling, Mobile Medical Services, OB/GYN, Outreach, Physical Exams, Primary Health Care, Referrals for Specialty Care, Shelter outreach, SOAR services, STD Testing, Street Outreach, Substance Abuse Treatment, TB screening/education, Tobacco cessation, Transportation services, Well Child Check-ups
Care Alliance works collaboratively with a number of health care and homeless service providers in the area, including AIDS Task Force, Catholic Charities, City Mission, Community Assessment and Treatment Services, EDEN, Lesbian/Gay Community Service Center, Lutheran Ministry, Mental Health Services, Neighborhood Family Practice, NEON, Office of Homeless Services, Proyectoluz, St. Malachi Center, Salvation Army, Transitional Housing, University Hospitals of Cleveland, Metro Health Systems, West Side Catholic Center, Y-Haven, and Departments of Health and Senior and Adult Services. Care Alliance is a member of the Ryan White Title I and Title II Consortiums, as well as several other community organizations and councils.
