CARE LINK – Community Medical Centers, Inc provides primary medical and behavioral health care services at local shelters, transitional/treatment programs, homeless services agencies and on the streets of San Joaquin County through its medical outreach program. Follow-up comprehensive primary care services are available to the homeless through the Gleason House Medical Clinic (located on the campus of the Stockton Gospel Center Rescue Mission) and all Community Medical Center sites.

Services

Medical & Behavioral Health services are provided at the Gleason House Medical Clinic at 423 S. San Joaquin St., Stockton, CA and all Community Medical Center site in San Joaquin, Solano and Yolo Counties. Services include primary medical care for acute/episodic and chronic care, well child exams, pediatrics, women’s health (Perinatal and Gyn), Podiatry, Chiropractic, Dental, laboratory, x-ray, pharmacy, Physical Therapy, nutrition counseling, medical outreach and case management.

Medical outreach services are provided through weekly visits to area shelters by an integrated medical/behavioral health and street outreach team. All services are provided at no-cost to the homeless.

Linkages

Substance abuse treatment services are accessed through local non-profits, faith based, shelters and other transitional programs. Mental health services are available from San Joaquin County Mental Health – Homeless Engagement and Response Team (H.E.A.R.T) and the Mental Health Crisis Clinic. Linkages for housing services include Central Valley Low Income Housing and San Joaquin Housing Authority along with emergency shelter providers. Care Link developed and coordinated the Stockton Community Coalition for the Homeless with more than 30 agencies actively involved and a Homeless Task Force that brought together agencies and policy makers in the city and county governments to address issues of homelessness. This group was reconvened through the efforts of Stockton City Councilwoman Susan Eggman and is now working to develop a 10 year plan to address homelessness. A significant collaborative effort since the program’s inception has been with the Stockton Gospel Cent er Rescue Mission. Together they are working to renovate the Gleason House, an early 1900’s era home located on the campus of the GCRM. By early 2011 the Gleason House will become the medical home for Care Link and homeless patients throughout San Joaquin County.
