Community Health Centers play a crucial role in filling the gap in access to health care for people experiencing homelessness. Direct Relief, in partnership with BD and the National Association of Community Health Centers (NACHC) has extended a Request for Proposal to support community health centers with innovative programs in Medication Therapy Management, services that optimize therapeutic patient outcomes. This initiative increases access to care and improves patient outcomes for underserved and vulnerable populations. Up to five $200,000 awards will be given for health centers to develop or improve current Medication Therapy Management programs in an effort to build awareness of CHC’s contributions in fulfilling community needs. The deadline to apply is May 4, 2018.
