Call for Applications Now Open: HCH Behavioral Health Integration Learning Collaborative

The National Health Care for the Homeless Council is excited to invite clinicians at health centers and organizations serving people without homes to join us in a new HCH Behavioral Health Integration Learning Collaborative. Please take part in this facilitated learning community consisting of six monthly one-hour sessions to learn best practices and share experiences in implementing integration of behavioral health care and primary care specific to the HCH community.

We are presenting this initiative in collaboration with partners across the country to meet shared objectives of creating a learning environment with peers and experts on behavioral health integration for people experiencing homelessness, to learn and implement best practices for behavioral health integration, and measure the impact of behavioral health integration on clients’ health outcomes.

Your participation is critical: learn more and apply to join this exciting new learning community. Applications are due by midnight CST on December 5, 2017.
