Numerous states and jurisdictions are implementing plans and strategies to provide opportunities for community-based housing with supports and services for persons moving from institutions and care facilities. Community health centers and behavioral health providers are key in the community service delivery system, but are often absent from the planning and coordination phase of this effort.
This webinar will address the findings of a survey conducted by CSH and the National Health Care for the Homeless Council to understand the role for health centers in serving the “Olmstead” and deinstitutionalized populations, and to highlight communities where health providers are actively involved.
