Recognition of housing as a social determinant of health is now widespread in the health care sector. Health centers, hospital systems, and managed care entities around the country are seeking new and innovative ways to partner with service and housing providers for their most vulnerable and costly patients. Along with these new partnerships, new funding mechanisms and resources have been identified from outside the mainstream housing and social services sector to expand these health and housing partnerships. Learn about communities that are doing this and how it has changed the face of housing.
