Santa Clara Valley Health—Valley Homeless Clinic is one of the sites participating in the National Health Care for the Homeless Council’s Health Care Innovation project titled Community Health Workers and HCH: A Partnership to Promote Primary Care. Since Khristine Quibilan joined the clinic as the community health worker, she has worked tirelessly to engage and connect clients with much needed services. Elizabeth Echeverria, who is serving as the CHW’s supervisor, has worked hand-in-hand with Quibilan and continues to volunteer a great deal of her time and expertise to this project. Together, they have helped facilitate incredible changes in the lives of their clients.
Among seven enrolled clients, emergency department visits have dropped from 203 visits six months before the intervention to 38 visits since enrolling in the project. The National HCH Council is proud to have Santa Clara Valley Health—Valley Homeless Clinic as a partner in this project and is excited to see the incredible work that will come from Quibilan and Echeverria over the next two years.
