Lincoln Community Health Center’s Healthcare for the Homeless Clinic in Durham, North Carolina is one of the sites participating in the National Health Care for the Homeless Council’s Health Care Innovation project, Community Health Workers and HCH: A Partnership to Promote Primary Care.
HCH Coordinator Julia Gamble, MPH, FNP, volunteered a great deal of her time to get this project up and running in Durham. During this process, Jen Robinson was hired as the community health worker. Jen has excelled in this position, quickly building her caseload and demonstrating an incredible work ethic. A unique asset to Lincoln’s HCH Clinic is its partnership with Housing for New Hope, particularly outreach nurse Marigny Manson, RN. Together, this team has already made lasting changes in the lives of the individuals enrolled in this project.
