For over a year, the National Health Care for the Homeless Council’s HCH Quality Leaders Initiative has studied the characteristics of high-performing HCH grantees and translated key practices into recommendations for fellow grantees. Along with publishing the full Case Study and Quick Guide, the Council has released two new practical tools to build upon this body of research.
The first—Improving Quality of Care: Clinician Tip Sheet—highlights the advice from staff of the HCH Quality Leaders regarding practices that can improve quality of care in a range of settings, geographies, and patient populations.
The second—Quality Improvement Resource List: Diagrams, Tools, and Publications—synthesizes an array of quality improvement tools available to aid HCH grantees in their quality improvement activities.
