The American Institutes for Research® (AIR), in its role as support contractor to CMMI, is pleased to announce that new and enhanced technical assistance is now available to help FQHCs achieve practice transformation and National Committee for Quality Assurance (NCQA) Level 3 recognition as Patient-Centered Medical Homes (PCMHs) by October 2014.
AIR is rolling out both national and local support networks to better serve you.
- At the national level—AIR’s partners at the National Association of Community Health Centers (NACHC), Qualis Health, and the MacColl Center for Health Care Innovation will continue to provide change concepts, learning events, quality improvement support, and coaching.
- At the local level—AIR has developed a “network of networks” through the collaborative efforts of your current Primary Care Associations (PCAs) . The network will deliver support adapted to your FQHC’s particular needs. Support may include coaching, learning communities, and help assessment tools to evaluate your center’s progress toward practice transformation.
The network of networks is organized around six regional clusters. Each cluster has a lead PCA that will work with partner PCAs to support all participating FQHCs in the region. The six clusters will be led by the PCAs in Missouri, Massachusetts, Florida, Colorado, California, and West Virginia (a list of participating States by cluster is attached to this email).
All technical assistance will be provided at no cost to your center through the FQHC APCP Demonstration. We encourage you to take advantage of any support that you feel will benefit your efforts towards achieving NCQA Level 3 PCMH recognition. In the coming weeks, you will be invited by our National Team and your PCAs to join learning communities and participate in calls, Webinars, meetings, and other events that will provide access to practice transformation resources.
