National HCH Grantee Directory
The HCH Directory is intended to foster collaboration and communication among federally-funded Health Care for the Homeless projects, and to provide a starting point to find health care resources for homeless people.
The following listings were updated in the fall of 2011, when each grantee was asked to provide updated information; some grantees did not provide updates, but the most recent information available to us is posted here. Grantees of the Federal Health Care for the Homeless Program (Section 330(h) of the Public Health Services Act, administered by the Health Resources and Services Administration) should review their listings below and make any necessary updates via this online form.
