The Colorado Coalition for the Homeless seeks a Program Manager to provide delivery and supervision of comprehensive, intensive, long-term clinical-based case management for clients involved within the Bridges team. The position reports to the Director of Mental Health and is located at 2111 Champa,Denver.

 

Bridges is a high intensity treatment team that utilizes the Assertive Community Treatment model in conjunction with the Mental Health Corporation of Denverto serve persons with serious and persistent mental illness. The interdisciplinary team employs community-based psychiatric treatment; housing; rehabilitation; case management; vocational; and other support services. Bridges services are tailored to assist individuals who will need ongoing clinical support and case management once they have been housed.

Qualifications

  1. Masters degree in counseling, psychology, social work or related field required.
  2. Current Coloradolicensure as a LCSW, LMFT or LPC required. Current Colorado licensure as a CAC II, CAC III or LAC strongly preferred.
  3. Demonstrated proficiency with motivational interviewing and program/staff management experience required.
  4.  Able to pass automobile insurance carrier’s motor vehicle record investigation.

Responsibilities

  1. Supervisors Bridges case managers, and co-supervises Bridges psychiatric nurses and psychiatrist, including assigning and monitoring of clients, and implementation of clinical based case management processes based on HITT Model, motivational interviewing and other evidence-based practices and principles.
  2. Facilitates daily team meetings and serves as a clinical resource to other team members.
  3. Coordinates and participates in the hiring, training, orientation, supervision, and evaluation of program staff,.
  4. Supervises referrals, intakes, assessments, ongoing services, and transitions of program clients.
  5. Provides direct services for homeless adults living with mental illness through outreach, crisis intervention, counseling, case management, milieu coverage and other service activities as necessary. Completes treatment planning, documentation of progress, service tracking, and outcome evaluation. Maintains confidential and complete client files.
  6. Acts as program liaison with MHCD and ensures compliance with all aspects of the MHCD contract.
  7. Acts as liaison to other external and internal program partners.
  8. Monitors baseline and ongoing data tracking for the purpose of assessing client progress and meeting ongoing reporting requirements.
  9. Coordinates and supervises program budget and related expenditures, including allocation of client needs funds.
