Job Description

Phoenix Health Center (PHC) for the Homeless Program seeks an ARNP to provide comprehensive, high quality primary and episodic and recuperative care at PHC sites, working within PHC’s clinical and administrative guidelines. PHC is one of seven sites operated by Family Health Centers, Inc., a Federally Qualified Health Center serving over 40, 000 patients annually.

Requirements

ARNP licensed to practice in the State of Kentucky Board certified in Family Medicine Interest in working within multidisciplinary teams to provide episodic and primary care to homeless persons.

Responsibilities

Responsibilities include coordinating patient care with a multidisciplinary staff at both the Healthcare for the Homeless clinic and outreach sites, including shelters, community kitchens, and on the street; referring patients when medically necessary for specialty, hospital, recuperative or other appropriate care; providing appropriate information for continuity of care. The Nurse Practitioner in this position primarily provide direct patient care with the remaining time devoted to other clinical administrative duties such as lab follow-up, quality assurance, and completion of patient forms.
