Project Description
The main focus of the Cheyenne Crossroads
Clinic is the provision of quality health care
for individuals and families who are currently
homeless in Laramie County. The clinic is staffed
weekdays and Thursday evenings by a mix of clinic
staff and volunteer doctors, nurses, and family
nurse practitioners.
Services
The Cheyenne Crossroads Clinic provides primary
health care with voucher referrals and transportation
to community providers for dental, mental health,
substance abuse, pharmaceuticals, and radiology
care. Although the clinic focuses on providing
quality primary health care, staff works with
various social service agencies to meet the
non-medical needs of individuals and families
who are homeless. Additionally, the clinic helps
both low-income and homeless individuals apply
to various pharmaceutical assistance programs.
Linkages
Other agencies and programs that are closely
involved with the Cheyenne Crossroads Clinic
include Alcohol Receiving Center, Chrysalis
House (alcohol and substance abuse treatment
center for women), Safehouse (emergency shelter
for women and children who are in an abusive
relationship), Comea House (emergency shelter
for those who are homeless), Department of Family
Services (public assistance and social services),
Departments of Vocational Rehabilitation, Developmental
Disability Programs, Medicaid and Medicare programs,
Head Start, Community Action of Laramie County,
AIDS programs, Wyoming Primary Care Association,
Wyoming Coalition of the Homeless, State of
Wyoming TB Program, City/County Health Department,
Veterans Administrating, and other local community
organizations, which include ministerial and
volunteer agencies.

Project Description
The 12th Street Clinic is administered by the
Community Action Partnership of Natrona County.
The12th Street Clinic provides primary health
care, outreach, case management, mental health,
substance abuse, and social services to individuals
and families at a social service center campus
near downtown Casper.
Services
The 12th Street Clinic provides a comprehensive
health care program to meet the unique needs
of individuals who are homeless. Outreach and
case management are provided and staff works
closely with other community agencies to ensure
that individuals benefit from available services.
Services include primary care, preventive, limited
dental and vision care, and medication. Mental
health and substance abuse counseling are provided
on-site through a contract with Central Wyoming
Counseling Center.
Linkages
The 12th Street Clinic has linkages through
qualified services organization agreements.
These agencies include emergency shelters, transitional
housing programs, substance abuse treatment
programs, emergency services programs, the local
housing authority, Community Health Center of
Central Wyoming, Wyoming Medical Center, WIC,
vocational rehabilitation, employment service
center, schools, Covering Kids Outreach, Department
of Family Services, and AIDS programs. The clinic
is active in the Casper Area Homeless Collaborative,
a regional continuum of care, and the State
Homeless Advisory Council.
