Project Description
Valley Health Systems (VHS) coordinates a
statewide Health Care for the Homeless program,
administered through five organizations located
in the cities of Huntington, Charleston, Wheeling,
Beckley, and Morgantown.
Services
VHS, in conjunction with the Cabell-Huntington
Coalition for the Homeless, provides direct
services -- via a team model -- out of Harmony
House and administrative oversight through subcontracts
with four other organizations. Harmony House
is a multi-purpose center for persons experiencing
homelessness. The health care team comprises
a nurse practitioner, counselor, medical director,
dentist, and social and outreach workers. Services
include primary medical care, substance abuse/mental
health assessment, counseling and referral,
transportation, dental care, showers, laundry,
and assistance in acquiring housing, employment,
and other social services.
Linkages
The project works closely with Cabell-Huntington
Coalition for the Homeless to arrange linkages
with other resources and services in Huntington.
