Project Description

The Fox Cities Community Health Center provides direct services and coordination of health services for homeless individuals who live in northeastern Wisconsin. Services are provided at the Emergency Shelter of the Fox Valley Region.

Services

Referrals to Fox Cities Community Health Center are received from the local emergency shelter and numerous temporary housing facilities. A nurse practitioner, mental health counselor, and outreach specialist provide direct services at the shelter facility. A full range of medical services are offered to individuals at the shelter. Transportation and support is also extended for after-hours and non-shelter clinical times. ThedaCare and Affinity Health Systems provide laboratory and radiology support. A broad array of social service support, including substance abuse assistance, is offered at the Emergency Shelter.

Fox Cities Community Health Center has linkages with ThedaCare Health System, Affinity Health System, University of WI Fox Valley Family Practice Residency Program, five City and County Public Health Departments, WI Well Woman Program, Family Services, Lutheran Social Services, Catholic Charities, Children's Society of WI, NAMI Fox Valley, Tri-County Community Dental Clinic, Valley Transit, Goodwill Industries of North Central WI, five nursing schools, WIC, and University of WI Nutrition Program.

Project Description

Health Care for the Homeless of Milwaukee (HCHM), provides directly and/or subcontracts for services for medical, dental, mental health, substance abuse, emergency housing, crisis intervention, prenatal care coordination, case management/protective payee, outreach, and advocacy. HCHM also provides HIV/AIDS prevention, testing, counseling, and residential services.

Services

HCHM provides a nationally-recognized street outreach program providing meals, tracking, transportation, and referrals to needed services. The program has been expanded to include intervention for custodial parents and children at a living site as a diversion to shelter placement. Direct services include mental health, case management for persons with severe and persistent mental illness, victims of domestic violence, and persons with HIV/AIDS, protective payee assistance, prenatal care coordination, transitional housing for adults living with persons with HIV, substance abuse counseling, and primary care subcontracted at five locations.

Linkages

HCHM has a presence at 23 locations throughout the Metropolitan area, including five HCHM-supported clinics. This model allows service coordination and access to pharmacies, laboratories, radiology, and specialists. Case managers/protective payee workers and medical social workers provide services in 13 sites. HCHM participates in the City of Milwaukee “Immunization 2000 Program,” and the Wisconsin “Healthy Start” and “Healthy Check” programs. Coordination of services is enhanced through participation in the Milwaukee County Shelter Task Force, which has developed a community-wide Continuum of Care process. Linkages are also with Wisconsin Primary Health Care Association, National HCH Council, Milwaukee County Behavioral Health Division, Milwaukee County Adult Services Division, Milwaukee County Housing Division, AIDS Resource Center/Wisconsin, Black Health Coalition, Wisconsin Department of Health and Family Services, and Wisconsin Primary Care Organization.

Project Description

North East Wisconsin (N.E.W.) Community Clinic provides health care services to individuals and families who are homeless in the Brown County area.

Services

Primary health care, case management, and outreach for substance abuse and mental health are provided in five area shelters and Salvation Army by a health care team which rotates among the six service sites. N.E.W. Community Clinic also operates a free clinic with volunteer physicians for persons who are uninsured to augment the health care services that are provided in the shelters. Referrals are made for dental care and outpatient AODA/mental health counseling. Cooperative agreements exist with local hospitals, physicians, family planning agencies, and others.

Linkages

N.E.W. Community Clinic is a member of Brown County Task Force on Homelessness and maintains affiliations with many local task forces and councils.
