Project Description
Community Health Association of Spokane (CHAS)
provides personalized, patient-centered care
to homeless individuals through an integrated
health care delivery model.
Services
CHAS provides primary medical, dental, nutrition,
behavioral health and pharmacy services at five
clinic locations in Spokane County. Services
include women’s health care, prenatal
and obstetrics, maternity support services,
health nutrition/education, well child check-ups
and immunizations, chronic illness management,
HIV/AIDS, outreach, case management, eligibility
assistance, transportation, translation, and
referrals to community resources as needed.
Linkages
The project maintains linkages with specialty
physicians, hospitals, community mental health/substance
abuse programs, homeless shelters, social service
providers, transitional housing units, employment
services, agencies in the private sector, universities,
schools, and churches. CHAS is a founding member
of the Spokane Homeless Coalition and participating
agency in developing Spokane’s Ten Year
Plan to End Homelessness.

Project Description
Community Health Center of Snohomish County
provides medical, dental, behavioral health,
pharmacy, and case management services to people
who are homeless in Snohomish County.
Services
Services are provided at clinic locations in
Everett and Lynnwood. Behavioral health specialists
are on-site at medical clinics to provide case
management and behavioral health care services.
A medical provider and medical assistant conduct
outreach to shelters and homeless service sites
in Everett. Buprenorphine-based substance abuse
treatment services are available for a limited
number of patients.
Linkages
Referrals are made for inpatient hospital care,
specialty services, most diagnostic services,
some pharmacy services, and radiology. Referrals
are made for housing, employment services, other
social services, and most substance abuse treatments.
Referrals for mental health services beyond
the scope of on-site mental health professionals
are made to the Regional Support Network and
Compass Health. Substance abuse referrals are
made to Catholic Community Services, Evergreen
Manor, Focus, SeaMar, and Stillaquamish Tribes.

Project Description
The Metropolitan Development Council (MDC),
a nonprofit community action agency, provides
family practice health services throughout Tacoma/Pierce
County.
Services
Primary, dental, and mental health care services
are provided via clinics in Tacoma Rescue Mission,
Community Health Care, and mobile medical/dental
units. An all-female staff provides weekly primary
care clinics for victims of domestic violence
and their children. Dental services are delivered
at homeless shelters, schools, and family support
centers via mobile units provided by Northwest
Medical Teams International, local dental providers,
and University of Washington School of Dentistry.
Substance abuse counseling, referral, outreach,
and case management are provided by MDC staff.
Linkages
MDC has a long history of cooperative planning
with other social service agencies. MDC has
memberships, partnerships, and linkages with
regional and State Coalitions for the Homeless,
Pierce County Continuum of Care, Tacoma Pierce
County Health Department, public/private schools
and universities, community mental health and
primary care clinics, and veterans' organizations.
Most recently, MDC was one of the lead agencies
participating in the development of Pierce County's
Ten Year Plan to End Homelessness.

Project Description
The Health Care for the Homeless Network (HCHN)
is a community project of Public Health - Seattle
and King County. Through a combination of direct
and subcontracted services, individuals who
are homeless can access care at shelters and
other homeless programs, community health centers,
and public health clinics.
Services
HCHN subcontracts with community based agencies
to operate multi-disciplinary, interagency teams
organized by geographic sub regions of King
County. Services are provided at more than 70
sites, including shelters, day centers, transitional
housing, a sobering support center, and community
health centers. Services include street outreach,
primary care, dental care, nursing care, mental
health and substance abuse services, case management,
linkages to housing, eligibility assistance,
a 22-bed medical respite program, and referrals.
HCHN provides technical consultation and training
for shelter staff on health and safety issues.
Services for people who are homeless are also
available at 10 public health clinic sites.
Linkages
Linkages are with numerous health, human services,
and government entities, including community
health centers, Harborview Medical Center, and
King County mental health and substance abuse
service system. HCHN is a member of State and
local homeless coalitions, a regional Committee
to End Homelessness, and Washington State Association
of Community and Migrant Health Centers. HCHN
collaborates with public health programs, including
primary care and dental clinics, TB clinic,
HIV/AIDS program, WIC, and family planning agencies.

Project Description
Sea Mar Community Health Center serves individuals
experiencing homelessness in Skagit, Island,
and Whatcom counties. Sea Mar provides direct
care as well as services subcontracted through
other community health centers.
Services
Sea Mar's HCH program operates within its clinic
facilities and through a subcontracted primary
care provider. All sites perform outreach to
shelters, missions, outdoor congregate sites,
labor camps, treatment centers, churches, and
other service agencies. All Sea Mar sites provide
primary care services, preventive health services,
well-child care, family planning services, urgent
care services, eligibility/entitlement assistance,
prescription financial assistance, transportation,
translation, and basic needs assistance. The
Skagit and Whatcom county clinics also provide
preventive dental services. Sea Mar also provides
mental health and substance abuse services,
including outreach, case management, counseling
(group and individual), and admissions to hospitals
and treatment facilities.
Linkages
Sea Mar makes referrals for inpatient care,
specialty services, most diagnostic laboratory
and radiology services, most pharmaceutical
services, housing, and vocational education
training. Referrals for substance abuse services
are made to Skagit Valley Hospital, North Sound
Evaluation and Treatment Center, Pioneer Center
North, Sea Mar Community Health Center's Inpatient
Treatment Centers, and St. Joseph Hospital.
Sea Mar also makes referrals to community service
agencies, food banks, and churches for financial
aid, legal assistance, social services, shelter,
and basic needs assistance. Referral agencies
include Community Action, Opportunity Council,
NW Justice Project, Allianza Pro Migrante, Salvation
Army, Red Cross, Friendship House Shelter, Lighthouse
Mission, Agape Mission, Women Care Shelter,
Skagit Domestic Violence and Assault Services
Shelter, LOVE In The Name of Christ (LOVE Inc.),
Neighbors in Need, Bellingham Food Bank, Helping
Hands, Viewcrest Assembly of God, Community
Covenant, Immaculate Conception, and St. Charles
Church. Sea Mar also does outreach at the Light
House, Agape Mission, and Women Care Shelter.

Project Description
Yakima Neighborhood Health Services (YNHS),
established in 1975, provides comprehensive
medical, dental, and public health to thousands
of disadvantaged residents each year. In 2004,
YNHS helped form the Homeless Network of Yakima
County to better address the needs of individuals
experiencing homelessness.
Services
YNHS provides primary medical and dental care
at its main clinic in downtown Yakima and at
Neighborhood Health-Neighborhood Connections,
a satellite clinic. Services are provided by
bilingual health professionals and support staff.
Pharmacy services are available through the
main clinic. Access specialists help patients
apply for SSI, food stamps, WIC, Medicaid, Medicare
Savings, Basic Health (WA State Subsidized Insurance),
and State Children's Health Insurance Program
(SCHIP). Pharmacy staff deliver medications
and supplies to the homeless clinic and shelters
as needed. An HCH outreach worker serves as
a “human recall system” to find
patients needing follow-up appointments. A public
health nurse rotates through a local community
shelter and public housing unit. Public health
services are provided through street outreach
in coordination with the local Projects for
Assistance in Transition from Homelessness (PATH)
program.
Linkages
The Homeless Network of Yakima County advocates
to improve quality of life, increase public
awareness, impact public policy, and prevent
and end homelessness. To that end, YNHS's linkages
include City of Yakima Neighborhood Development,
Community Volunteers, Consumer Credit Counseling,
Diocese of Yakima Housing Services, Region 2
Child & Family Services and Juvenile Rehab
