Project Description
The Peninsula Institute for Community Health
is a private, nonprofit community health center
that provides primary health care services for
medically indigent persons who are homeless
in Newport News and Hampton.
Services
The Health Care for the Homeless Program provides
direct medical care and case management. Primary
acute care and prevention are the focus of the
medical team, comprising a physician, nurse
practitioner, physician's assistant, and case
managers. The program provides services to people
who are homeless through the use of Hampton
University Nursing Center mobile health van
and a treatment center. In addition, an evening
clinic is held at a local shelter. Ongoing health
care and specialty medical services are provided
through referral to participating health centers
and doctors. Services include physicals, dental,
prescriptions, pregnancy tests, and assistance
with applications for food stamps, Social Security
disability benefits, etc. Referrals are made
for mental health and substance abuse services.
Case managers advocate for clients, set up initial
appointments, track health records, provide
transportation, and monitor transfers.
Linkages
The HCH project maintains a cooperative and
collaborative relationship with all of the major
public and private agencies serving people who
are homeless. In addition, cooperative relationships
exist with substance abuse and mental health
treatment facilities, Redevelopment and Housing
Authority, HomeBase of Virginia Peninsula, Virginia
Peninsula Council on Domestic Violence, Residency
Program, employment security, Social Security,
and others. Affiliation exists with Virginia
Coalition for the Homeless through which educational
efforts, directed toward both general and homeless
populations, are conducted. The public is informed
of issues regarding people who are homeless
through letters, health fairs, and speaking
engagements. Clients are educated in areas of
concern to them, including AIDS, personal hygiene,
legal issues, prenatal care, and nutrition.

Project Description
Portsmouth Community Health Center operates
two health care centers to provide a full range
of primary health care services to homeless
adults and children in the cities of Norfolk
and Portsmouth.
Services
The Park Place Medical Center, located in Norfolk,
and Portsmouth Community Health Center, located
in Portsmouth, provide comprehensive primary
health services and case management on-site.
Primary acute care and prevention are the focus
of the medical team, comprising a physician,
nurse practitioner, licensed practical nurse,
and pharmacy technician. The homeless program
gives the hospitals in the community a place
to direct patients for a medical home. The medical
home constitutes the delivery of primary care
partnerships with patients and community providers
to access a comprehensive array of services.
Patients have a shorter wait time for appointments,
compared to the hospital clinics. They also
have a medical home with appropriate follow-up
and help securing needed prescriptions. Patients
also have access to case management. All these
services are under one roof.
Linkages
Linkages include medical and social service
agencies, including YWCA, Norfolk Department
of Human Services, St. Columba's Medicine and
Transportation Program, Tidewater AIDS Crisis
Taskforce, Norfolk Redevelopment and Housing
Authority, local Social Security Administration,
Norfolk Homeless Consortium, and Portsmouth
Homeless Advisory Consortium.

Project Description
The Daily Planet HCH project provides a full
range of services to individuals who are homeless
in Richmond.
Services
Services are provided by a physician, registered
nurse, dentist, psychiatrist, mental health
therapist, substance abuse counselor, social
work managers, and outreach workers. Primary
care includes physical exams, gynocological
exams, screening for hypertension, diabetes,
TB, HIV, STDs, and urgent care. Treatment and
follow-up is provided for chronic conditions
such as hypertension and diabetes. The dental
health clinic provides preventive and restorative
dental care. Health education includes preventive
health care, nutrition, chronic disease management,
dental care, and mental health care. Mental
health services include therapy, medication
management, and case management. Substance abuse
services include group and individual counseling,
as well as access to inpatient treatment.
Linkages
Linkages include medical and social service
agencies. The HCH program participates in Richmond
Enhancing Access to Community Health Care (a
coalition of safety net providers) and Homeward
(a collaboration of homeless service providers).
The HCH program provides services to the Greater
Richmond Medical and Mental Health Respite Program.
The program also works closely with the Medical
College of Virginia Hospital (for specialty
care) and area schools and universities (to
provide internships and externships).
