Project Description

Centro San Vicente Homeless Clinic, a satellite clinic operated by Centro San Vicente, is located within the Opportunity Center where Myrtle Street Shelter serves as a multi-purpose shelter for individuals who are homeless.

Services

Referrals to the Centro San Vicente Homeless Clinic are received from 30 shelters, six orphanages, and numerous housing facilities throughout the community. A nurse practitioner, nurse, medical assistant, and pharmacy clerk staff the homeless clinic. A full range of primary medical services are available to all individuals attending the clinic. The clinic is open full-time, 5 days a week. Transportation, provided by the Opportunity Center, is also available. Providence Memorial Hospital subsidizes the laboratory and pharmacy services. Various support services offered by the Opportunity Center provide food, shelter, and behavioral health and substance abuse counseling. Clients are referred to other services at the Opportunity Center or to other providers within the Homeless Coalition. Outreach programs within the community are regularly scheduled.
In 2005, the grantee was awarded a HRSA Mental Health Expansion grant for adult counseling and case management services. A grant was also received from the U.S. Department of Housing and Urban Development (HUD) Continuum of Care for these services. The grantee partners with the Opportunity Center for the Homeless and area shelters for this program.

Project Description

CentroMed, formerly El Centro Del Barrio, operates nine HCH clinics within or adjacent to homeless shelters. The clinics provide homeless individuals and families with primary health care, dental, social services, child development services, substance abuse counseling, health educational group sessions, and referrals as needed to other Continuum of Care agencies. Street outreach and collaborative health fairs also make services available to homeless persons not in shelters.

Services

The program provides comprehensive primary medical care, dental care for adults and children, social services, case management, counseling, substance abuse prevention, education and counseling, child development screenings, psycho-educational group services, and outreach. Referrals include mental health services, substance abuse treatment, podiatry, vision care, and specialty medical care. Health educational group services cover anger management, parenting, substance use risk education, HIV/AIDS risk education, Hepatitis/STD's, diabetes, and nutrition. Clinics are located in the San Antonio Metropolitan Ministries (SAMM) Shelter, Dwyer Transitional Shelter, Children's Shelter of San Antonio, Infants' Shelter, and Kinetic Concepts, Inc (KCI) Residential Treatment Center, Respite Care of San Antonio Davidson House shelter and daycare, and La Paloma de Paz family violence shelter.

On-site services are available in clinics located in the Children's Shelter, the SAMM Shelter, Battered Women's Shelter, Infant Shelter, and Dwyer Transitional Shelter, and Respite Care of San Antonio. Dullnig House Clinic is located where large numbers of persons who are homeless congregate and is adjacent to the Salvation Army Shelter.

CentroMed collaborates with the following organizations in the delivery of health care services: Center for Health Care Services, American GI Forum, HCH Veterans, Salvation Army Adult Rehabilitation, Easter Seals, Any Baby Can, ophthalmologist Dr. Robert Rice, Prevent Blindness, area homeless service providers, and HIV service providers. In addition to links to the Metropolitan Health District and the Bexar County Hospital System, the agency also participates in the shared services agreements developed through the Texas Association of Community Health Centers.

Project Description

The City of Austin operates a multi-site HCH project that provides comprehensive services in Travis County. The project's various sites, modes of service delivery, and community partnerships target services to three distinct subpopulations: single adults, unaccompanied youth, and women and children who are victims of domestic violence. The project is managed as part of the Community Health Center system operated by the City of Austin.

Services

The HCH project serves single adult men and women at its clinic within the Austin Resource Center for the Homeless (ARCH), a low-demand shelter and day resource center in downtown Austin. In addition to primary care, the ARCH clinic provides integrated psychotherapy and intensive case management, providing linkages to a wide array of housing, legal, substance abuse treatment, and other services offered by co-located agencies. The project provides primary care to a smaller number of single adult men, women, and families at its clinic in the Salvation Army shelter. It provides medical care to women and children at SafePlace, a shelter for victims of domestic violence, and to unaccompanied youth at LifeWorks outreach center. Dental care is provided via the City of Austin's mobile dental units. Vision exams and eyeglasses are provided at a church facility. Pharmacy services are provided through the Community Health Center pharmacy program. The City of Austin's longstanding relationship with the local mental health center allows for close coordination between HCH staff and the mental health center's ACCESS program. An ACCESS psychiatrist treats patients 2.5 days per week at the ARCH clinic.

Project Description

The Community Health Center of Lubbock (CHCL) has been a Health Care for the Homeless grantee since 1994. CHCL provides comprehensive health care and social services to individuals who are homeless in Lubbock. This is accomplished through a network of collaborating agencies and other health care providers. CHCL provides primary and preventive care while working closely with other agencies to find needed resources. CHCL has a vision clinic on-site. CHCL also serves as a clearinghouse for multiple services for persons who are homeless. CHCL's goal is to treat persons who are homeless with dignity and respect and to meet their diverse needs in a holistic manner.

Services

The CHCL provides services both on-site and off-site at four other satellite clinics. On the premises, staff provides in-depth assessment, information, referral, and case management services. Medical services are provided by full-time physicians and include primary care for adults and children, prenatal, obstetrics and women's health care, dental services, and vision services. Social services include food and clothing vouchers, hygiene boxes, rent and utility subsidies for marginally-housed or at-risk clients, bus passes, and health education. Group, family, and individual counseling, nutritional supplements, and health care screenings are also available.

CHCL has developed extensive linkages with local agencies and other health care providers in the community, including Lubbock Regional MHMR (substance abuse and mental health services), Covenant Community Outreach (health education, mammography services, counseling, nutrition services), United Coalition Pharmacy (prescription services), Neighborhood House (emergency assistant services, including food voucher and utility assistance), Heartline (clothing assistance), Salvation Army (rent and/or mortgage assistance, emergency shelter, hot meals), First United Methodist Church (hot meals), Lakeridge United Methodist Church (hygiene boxes), and Family Promise (transitional housing for families).

Project Description

Homeless Outreach Medical Services (HOMES), a program of Dallas County Hospital District (dba Parkland Health & Hospital System), uses a combination of fixed and mobile clinic sites to bring health care to individuals who are homeless. The clinics operate on a walk-in basis. Individuals of all ages receive high quality, comprehensive primary health care services, including acute, chronic, and preventive care.

Services

The clinics have class D pharmacies and point-of-care laboratory testing available on-site. The main campus of Parkland Health & Hospital System supports the clinics with a full range of laboratory, radiology, pharmacy, and subspecialty services. In addition to medical services, HOMES offers services of a podiatrist, social workers, dentists, dieticians, psychologists, and substance abuse counselors. Residential drug treatment is provided by Turtle Creek Manor.

The Harris County Hospital District (HCHD) Health Care for the Homeless Program (HCHP) uses health care teams to provide on-site and mobile health care, mental health services, dental services, and substance abuse services at homeless shelters and day centers throughout Harris County.

Services

HCHP provides on-site services at shelters and day centers where individuals who are homeless seek other services. Five health care teams with attached eligibility workers, social workers, mental health counselors, a dental care team, and a substance abuse counselor provide health care, dental care, case management, eligibility assistance, and substance abuse and mental health counseling at homeless shelters and day centers. Two mobile medical units provide outreach to unsheltered homeless individuals.

Project Description

HCH services are provided by Regence Health Network (RHN), an integrated network of Federally Qualified Health Centers, providing primary health, dental, and behavioral services throughout the South Plains and Panhandle of Texas. The homeless clinic is co-located at the Amarillo United Way Resource Center in Amarillo.

Services

The homeless clinic provides a comprehensive array of primary and preventive health care services, including women's health (OB referrals, family planning), pharmacy, pediatric services, immunizations, Texas Health Steps, other well-child services, geriatric and adult chronic disease management and prevention, and lab services. Clinic staff, including a full-time family nurse practitioner, provide medical care at shelters, including Martha's Home, Another Chance, City of Refuge, Downtown Women's Center, Domestic Violence Center, Faith City Mission, Haven, and Salvation Army. Street outreach is conducted on a regular basis. Clients may also take advantage of the Day Center as well as other services provided in the Tyler Resource Center.
