Project Description

Falls Community Health is a nonprofit clinic that provides complete primary health care and dental care for all age groups. Anyone can be a patient at the clinic. Our goal is to provide an open door to medical and dental care.

Services include acute care, chronic health care, health maintenance and physical exams, lab and x-ray services, well child care and immunizations, family planning services, patient educationa nd disease prevention, mental health counseling, confidentail HIV testing and care. Routine dental care, emergency dental care and fillings, cleanings, and extractions.

FCH refers to private and public agencies whenever services aren't available in the clinic.

Project Description

Community Health Center of the Black Hills Outreach Program is a project of the Community Health Center of the Black Hills, Inc. The program's main office is located at the Cornerstone Rescue Mission with outreach sites at the mission, as well as Working Against Violence Shelter, Salvation Army, Cornerstone Rescue Mission Women and Children's Shelter, and Cornerstone Rescue Mission-Men's.

Primary health care services are provided at all sites. The sites are staffed by a medical provider, nurse, and case manager/mental health counselor. Substance abuse counseling is provided in conjunction with City County Alcohol and Drug Center. Additional service needs are met through referral to the main site of Community Health Cen
