Project Description
Rapid City Community Health Center Outreach
Program is a project of the Rapid City Community
Health Center. The program's main office is
located at the Cornerstone Rescue Mission with
outreach sites at the mission, as well as Working
Against Violence Shelter, Pennington County
Community Action Project, Salvation Army, Cornerstone
Rescue Mission Women and Children’s Shelter,
and Cornerstone Rescue Mission-Men's.
Services
Primary health care services are provided
at all sites. The sites are staffed by a full-time
PA-C, medical clerk, and case manager/mental
health counselor. Substance abuse counseling
is provided in conjunction with Pennington County
Alcohol and Substance Abuse Center. Additional
service needs are met through referral to the
main site of Rapid City Community Health Center
and other agencies.
Linkages
HCH refers to private and public agencies whenever
services aren't available in the clinic.
