Project Description
Crisis Ministries' (CM) Health Care for the
Homeless project provides a primary health care
clinic 5 days a week and mental health and substance
abuse services to any person who is homeless
in the greater Charleston area. All services
are provided at Crisis Ministries which operates
the largest shelter program in South Carolina.
Services
Crisis Ministries operates a night time only
Men’s Shelter, a Family Shelter/Day Center
for single women and families, a 7 days/week
soup kitchen serving free hot lunches to anyone
who is hungry, full case management services
including mental health and substance abuse
services, an HCH Health Clinic, and a small
respite care program.
Linkages
Informal linkages include the Charleston County
Health Department, Roper Hospital, Medical University
of South Carolina and its College of Nursing,
Charleston County Emergency Medical Services,
and East Cooper Community Outreach for dental
services.

Project Description
Little River Medical Center Health Care for
the Homeless program provides primary care services
to individuals who are homeless in Horry County.
This service is delivered via a free-standing
health center model, outreach at area shelters,
a food kitchen, and a mobile medical van.
Services
The HCH program is operated by a full-time,
family practice, bilingual physician, a medical
social worker (trained to provide mental health
and substance abuse treatment), an outreach
van/driver, and support staff. Dental services
are provided at two dental sites. The program
operates Monday through Friday in a center located
near homeless shelters and on designated days
at area shelters and a food kitchen. The program
collaborates with area agencies and resources
to help provide a continuum of care for individuals
who are homeless.

linkages with Shoreline Behavioral Health, Waccamaw
Public Health District, Horry-Georgetown Technical
College, Total Care for Homeless Coalition,
SOS Healthcare, Conway Hospital, Community Kitchen,
Carolina Health Specialist, City of Myrtle Beach,
Home Alliance, Grand Stand Housing, Horry County
Shelter Home, Street Reach Ministries, Myrtle
Beach Haven, Citizens Against Spousal Abuse,
and Sea Haven.

Project Description
To effectively meet the needs of the homeless
population across a 13-county region of upstate
South Carolina, New Horizon Family Health Services
of Greenville has formed a partnership with
Carolina Health Center in Greenwood, ReGenesis
Health Center in Spartanburg, and the Upstate
Homeless Coalition to develop an HCH Project.
Primary care and other services are delivered
at clinic sites, through mobile medical and
outreach approaches using Caring Attitudes,
Respect, and Empowerment Teams (C.A.R.E.).
Services
The primary method of service delivery are
interdisciplinary C.A.R.E. Teams that deliver
primary medical care, disease management, health
education, medication assistance, mental health
services, substance abuse services, street outreach,
and case management at multiple locations and
through the mobile medical C.A.R.E. Express.
Linkages
Linkages include AID Upstate (AIDS Service
Organization), Upstate Homeless Coalition, Greenville
County Health Department, Greenville Free Medical
Clinic, Mental Health Centers, Phoenix Center
(SC Department of Alcohol and Drug Abuse Services),
specialty care providers, homeless shelter providers,
church homeless feeding programs, Department
of Social Services, United Way of Greenville
County, local detention centers, and hospitals.

Project Description
Richland Community Health Care Association's
Health Care for the Homeless program has been
in operation since 1988.
Services
Services include intensive case management
and outreach efforts, direct medical treatment
through the project’s medical, nursing,
and social work staff, and coordination with
local agencies. The project serves as a clearinghouse
of information to assist public, private, and
governmental organizations.

The project has linkages with Midland Homeless
Consortium, Columbia Area Mental Health (CAMH),
Lexington/Richland Alcohol and Drug Abuse Council,
Cooperative Ministries, United Black Fund, Salvation
Army, Oliver Gospel Mission, Richland County
Department of Social Services, South Carolina
Department of Health and Environmental Control,
Palmetto AIDS Life Support Services (PALSS),
Palmetto Health Alliance, and other public and
private service providers.
