Project Description
For the past 35 years, Migrant Health Center
(MHC) has provided accessible comprehensive
health, dental, and social services to the 15
municipalities comprising the western region
of Puerto Rico. The center has six clinics strategically
located in hard to reach underserved communities.
Services
Services include medical evaluations, OB/GYN,
oral care, pediatrics, mental health, evaluations
by nutritionist and social worker, on-site laboratory
and pharmacy, health education, HIV counseling
and testing, outreach activities for high risk
populations, and HIV prevention services for
intravenous drug users. MHC works in collaboration
with the Puerto Rico Department of Health and
its Regional Immunology Clinics. Referrals to
substance abuse treatment, emergency health
care, and other services are coordinated through
a long list of agencies that have collaborative
agreements with MHC. In 2002, MHC received a
grant award to implement the integration of
buprenorphine opioid abuse treatment into the
HIV primary care setting.

Project Description
The San Juan HCH Program is a comprehensive, multidisciplinary primary health care program, specialized in serving homeless individuals in the City of San Juan, as part of the San Juan Integrated Health System. The project supports a non-prejudiced, unconditional, no barriers approach to care with one-stop service, short term appointments, stability of providers, and street outreach.
Services
Through the SJHCH Program comprehensive and preventive primary health care, several services are provided to the city's homeless, such as health maintenance, TB, cancer and dental screening, onsite STD treatment, immunizations, wound care, blood/urine specimen collection, health education, psychosocial needs assessment, substance abuse/alcohol counseling, case management/enabling services, networking, advocacy, tracking and community outreach. Referrals are provided for drug/alcohol programs, additional diagnostic work-ups and other specialized services within the Main Site Center, the San Juan Health Department and other health agencies. Continuity of care is achieved by assuring the consistency of the provider in charge of the participant's care: assigning specific staff to deliver the services on regular schedule basis; working closely with the facility (site) staff to deliver the services and developing systems that respond to the facility's functions an d utilization by the participants. The Program has expanded its health care services hours from 5 to 8pm in two (2) of the sites and with the recent acquisition of a mobile unit we are accessing areas where homeless persons congregates.
Linkages
The SJHCHP provides preventive and comprehensive primary health care in 3 sites, through Memorandums of Understanding (MOU)with La Fondita de JesĂs (soup kitchen), La Casa de Nuestra Gente (transitional shelter) and Centro de Acogida (transitional shelter). Other linkages are Pueblito de Jesus (shelter), Iniciativa Comunitaria (soup kitchen), Casa Julia de Burgos (domestic violence women's shelter), Salvation Army (emergency shelter), Hogar El Buen Pastor (transitional shelter), Casa Rosa I (transitional housing), Casa Rosa II (permanent housing), Albergue El Paraiso (shelter), Hogar Esperanza (transitional housing), Estancia Corazon (soup kitchen), Hogares CREA (Drug Rehabilitation Treatment), San Patricio Mental Health Center, Programa Más Salud Mente Sana (San Juan City Mental Health Program), San Juan Municipal Hospital, and San Juan Aging Center.
