Project Description

Community Health Net (CHN) is the only Federally Qualified Health Center in Erie County providing a medical safety net to approximately 15,000 uninsured and underinsured individuals.

Services

CHN provides primary medical, dental, obstetrics and gynecology, vision, and HIV/AIDS services to persons living on the street and residents of shelters located within Erie County. CHN offers transportation, a Prescription Assistance Program, and translation services. A mixed case management model is used. Outreach mechanisms have been established through linkages with shelters, soup kitchens, social service providers and community-based organizations. The health care team is composed of primary care providers, dentists, and nurses. A high priority is placed on health risk behaviors, education and early detection, control and surveillance of vaccine preventable illnesses, enhanced nutritional standards, HIV/AIDS treatment and counseling, substance abuse and mental illness risk reduction, identification and intervention, and improved access to reduce tooth loss and dental disease. Laboratory, radiology, pharmacy, emergency medical service, inpatient care mental health service, substance abuse services, crisis intervention, and supplemental assistance are available through referral arrangements with other area providers.

To ensure that needs are met, CHN collaborates with the Erie County Homeless Impact Committee, Greater Erie Community Action Committee Substance Abuse Program, Erie County MH/MR, Erie County Department of Health, St. Paul Lutheran Church Upper Room, The Sisters of St. Joseph, Community Shelter Services, Gateway Erie, Erie Criminal Justice System, Erie City Missions, Erie Salvation Army, Erie County Department of Public Welfare, Gaudenzia Crossroads, Community of Caring, and Mercy Center for Women.

North Side Christian Health Center (NSCHC) is a comprehensive primary care community health center providing services to the medically underserved and uninsured.

Services

Primary Health Care for the entire family including office visits, 24-hour on call emergency coverage, pediatric specialty, gynecologic services, laboratory studies, Hepatitis C Clinic, substances abuse treatment for opioid addiction, behavioral health counseling, patient advocate, and pharmacy program.

Project Description

Philadelphia's Health Care for the Homeless Program, administered by Public Health Management Corporation, provides primary health care, social services and behavioral health care in Philadelphia, and through sub-contract in Delaware County. Outreach is provided at 40 shelters, day programs and other facilities that serve the homeless. Comprehensive physical health, behavioral health, family planning and social services are provided at the Mary Howard Health Center, located in center city Philadelphia. Additional HCH sites where primary care is delivered are the Stenton Family Manor and People's Emergency Shelter, both in family shelters. Under HCH are two primary care in public housing sites: The Rising Sun Health Center and the PHMC Health Connection. Within the last year HCH added the Care Clinic, a primary care site for people with HIV/AIDS.

Services

HCH provides family planning, behavioral health and social services at shelter-based clinics as well as at HCH health centers and public housing sites. The project is part of the Governor's Diabetes Collaborative. Optometric services and nutritional services are also provided. Dental is NOT provided on-site.

Linkages

The project has linkages to all major City departments, substance abuse programs, HIV/AIDS organizations, the Family Planning Council, hospitals, domestic abuse programs and homeless and housing advocacy programs. Staff have knowledge of (and often linkage to) transitional and permanent housing programs, food cupboards and other services.

Project Description

Health Care for the Homeless (HCH) serves a predominately urban population in the Greater Pittsburgh area, extending to the Mon Valley and Shenango Valley, with a large network of health professionals.

Services

HCHP provides health and health related services through on-site clinics at shelters, drop-in centers, transitional/bridge housing programs, and soup kitchens. Services include primary care, needs assessments, pharmacy services, gynecology, pediatrics, behavioral health including substance abuse, podiatry, dental, vision, and referrals to specialty services.

Linkages

HCHP is part of a network of community based organizations serving individuals who are homeless. HCHP maintains strong working relationships with the Allegheny County Homeless Alliance, Allegheny County Department of Human Services, Salvation Army, various academic institutions and hospital systems, and numerous soup kitchens, shelters, and human service agencies.

Project Description

Rural Health Corporation (RHC) serves a predominantly urban population of individuals and families who are homeless in Wilkes-Barre and the surrounding Greater Wyoming Valley in Luzerne County.

Services

RHC provides primary care services as well as basic laboratory and pharmaceutical services at the RHC McKinney Clinic located at the St. Vincent de Paul Soup Kitchen in Wilkes-Barre. The service delivery model is a clinic setting incorporated into a multi-service center. Three clinic sessions are held each week, staffed by physicians, mid-level practitioners, nurses, mental health/substance abuse outreach workers, and other support personnel, including volunteers. Residents from Wyoming Valley Health Care System's Family Practice Residency Program also provide patient care services as part of their community medicine rotation.

Linkages

RHC is affiliated with Wyoming Valley Health Care System (WVHCS) through which emergency and inpatient hospital services are provided. Through linkage with WVHCS, additional support is provided for laboratory and pharmacy, specialty care, and diagnostic services. Other linkages include HelpLine (24-hour service for outreach and coordination of after hours assistance), Catholic Social Services (food, clothing, emergency shelter, and preparation for independent living for adolescents), Commission on Economic Opportunity (case management, housing, and benefit assistance), Domestic Violence Service Center (emergency shelter and transitional living for women and children), and Salvation Army/Kirby Family House (transitional living for families).
