Project Description
Central City Concern (CCC) was founded in 1979 to provide pathways to self-sufficiency through active
intervention in poverty and homelessness. CCC services have developed to include a continuum of care, including
health care, chemical dependency treatment, mental health services, case management, special needs housing, very
low income housing, workforce development, and employment services.
Services
Central City Concern health services provides primary care that is integrated across several dimensions at the Old Town Clinic. Services include allopathic & naturopathic primary care,
complementary care, mental health services, and recuperaive care. Urgent dental care is provided by volunteer services from Medical Teams International. CCC has multiple programs for outpatient mental health and chemical dependency treatment, case management, recovery mentoring,
outreach, and supportive housing and employment services. CCC provides medically monitored detoxification and outreach services at the Hooper Center. Agreements with area hospitals for uninsured and
managed care contracts for Oregon Health Plan clients provide access to specialty providers and hospital care. CCC
also provides 1,309 units of low-income housing, including 934 units of single and family alcohol and drug free
community housing units (ADFC). CCC also provides job training, development, and placement services.

Project Description
Safe & Sound (S&S) uses a multidisciplinary
team approach to provide medical, mental health,
and substance abuse
treatment within wraparound services, including
shelter, transitional housing, education, employment,
and assistance
to meet basic needs. Once a relationship is
established with staff, youth develop individualized
plans and identify
useful tools. Services are accessible through
any of eight partner doors. The S&S clinic
is a satellite of Community
Health Centers of Lane County.
Services
Safe & Sound Homeless Youth Clinic provides
medical, mental health and substance abuse services
for homeless,
runaway, and at-risk youth. In addition, youth
receive sexual assault support services, HIV
education/testing/counseling, health and safety
education, shelter, transitional housing, case
management, and other
supportive services. The day access center provides
forums for youth to access basic needs, such
as food, clothing,
showers, and telephone.
Linkages
Safe & Sound has received considerable
support from many sectors of the community,
including the government,
health professionals, hospitals, businesses,
foundations, and other social service organizations.
S&S works with
Lane County Child and Adolescent Behavioral
Health, Department of Public Health, and PeaceHealth
Hospital. In
addition, S&S provides a balanced approach
to working with youth that includes businesses,
public safety, and
social service agencies working together to
address the complex issues of homelessness.

Project Description
La Clinica provides year round outreach services to the
homeles community through partnerships with local homeless shelters and the
Jackson County Homeless Taskforce, and in coordination with local city
jurisdictions, school districts, and social service providers. La Clinica's mssion is to provide quality medical, dental and social services to the
diverse populations witin their communities, especially to those who face
barriers to health care.

Project Description
The Multnomah County Health Department operates the Westside Health Center, a large primary care clinic located in downtown Portland. It also provides services to a satellite clinic located within a local dining hall in Southeast Portland. In addition, the Health Care for the Homeless Program also operates a smaller primary care clinic, La Clinica de Buena Salud which targets health care services to homeless families in a largely Hispanic Community.
Services
The HCH has strong linkages with the Coalition of Community Clinics, the safety net clinics. We have worked with Corrections Health to take discharges directly from the County Jail and prisons. The clinic also accepts weekly referrals from ten local agencies. The satellite clinic is located in the St. Francis Dining Hall, which also houses a social worker.
Linkages

Project Description
The Native American Rehabilitation Association
of the Northwest (NARA-NW) provides integrated
health, mental
health, substance abuse treatment, and social
services to Native American and Alaska Native
people who are
homeless or have low incomes. NARA-NW has been
providing services in the Portland area since
1970. NARANW
primarily serves people living in Oregon and
Southwest Washington. The NARA-NW community
health center
operates one primary health clinic, a satellite
clinic within the residential substance abuse
treatment center, an
outpatient mental health center, an outpatient
substance abuse treatment center, a residential
substance abuse
treatment center, and a small pharmacy.
Services
NARA-NW provides health, mental health and
substance abuse treatment services that are
culturally specific to the
needs of American Indian and Alaska Native people.
These services are often coupled with social
services and
supports that enable access to services such
as case management, transportation, and housing
assistance. NARANW
has more than 100 staff deployed across four
sites and in the community.

NARA-NW has linkages with area schools, the
region's Departments of Health, other community
health centers,
substance abuse treatment providers, and homeless
services providers. NARA-NW also has linkages
to the region's
tribes and other Urban Indian programs.

Project Description
Northwest Human Services (NWHS) serves individuals
who are homeless in Salem through outreach services
provided by its Homeless Outreach and Advocacy
Project and by providing medical services at
its West Salem
Clinic, a community health center.
Services
The West Salem Clinic and the Total Health
Community Clinic provide primary health care
and mental health care,
using the services of a health care team comprising
physicians, physician's assistants, nurses,
counselors, and
outreach workers. In addition to providing primary
care, NWHS serves as the entry point for social
services for
persons who are homeless. Additional components
of the NWHS program include the Homeless Outreach
and
Advocacy Project (coordinates outreach and comprehensive
mental illness services, including dual diagnosis
treatment), Homes Offering Shelters to Teens
(assists runaway and homeless youth and their
families), and Crisis
and Information Hotline (provides 24 hour information,
referral, and crisis counseling services).

Linkages are in place with other agencies to
provide substance abuse treatment.

Project Description
Outside In uses interdisciplinary and alternative
medicine to reach the hardest to serve homeless
and marginalized populations, including homeless youth, injection
drug users, Latino day laborers, and sex workers.
In 2005, the clinic was dually funded as a Community Health
Center. The agency also operates extensive homeless
youth social services, including mental health counseling,
housing, drop-in, employment services, and an
alternative school.
Services
Outside In provides primary care and health education. Through staff physicians and partnerships with medical schools, Outside In provides allopathic, naturopathic, and Oriental medicine and acupuncture. The clinic offers mental health and substance abuse services to its established clients through primary care integration and specialty mental health services. Dental care is provided by volunteer dentists through contract with Medical Teams International using their mobile dental van. Outside In's medical director and attending physicians oversee medical residents and 4th year naturopathic students in providing services. Staff MDs have admitting privileges at Oregon Health and Sciences University Hospital. Staff at Outside In does extensive outreach, referrals, and social work wrap-around services. 50% of support staff are bilingual English/Spanish and provide interpretation services. All other interpretation provided through contract by a phone interpretation co
mpany. Outside In has a small pharmacy located on site for it's established patients. The Outside In clinic has mutual referral relationships with other safety net clinics and a referral network of private medical specialists who see patients free of charge. The program conducts medical outreach using a full-service mobile medical van equipped with two exam rooms, waived lab, bathroom, and charting area. Outside In also operates a tattoo removal program for gang-involved youth. The agency operates 2 Dog Day-Care Centers - an employment training program for homeless youth and social enterprise generating revenue.
Linkages
Outside In has reciprocal referral relationships
with Multnomah County's Downtown Clinic and
Old Town Clinic, both of which serve low income and homeless
adults. Outside In also has referral arrangements
with the Native American Rehabilitation Association which serves
Native Americans of all ages.

Project Description
White Bird Homeless Health Care (WBHHC) project
serves people who are homeless in the Eugene-Springfield
metropolitan area.
Services
WBHHC provides primary medical and dental services,
including immediate, ongoing, and preventive
care, at a joint facility centrally located in Eugene.
Basic laboratory work and medication are provided.
Access to specialists, diagnostic services, and hospital admissions
are arranged. Staff includes physicians, dentists,
dental assistants, a psychologist, a hygienist, nurses, counselors,
EMTs, acupuncturists, and mental health, crisis,
and substance abuse workers. WBHHC provides 24-hour crisis counseling
as well as screening, evaluation, and referral
for mental health and substance abuse services. The project’s
Crisis Assistance Helping Out On The Streets
(CAHOOTS) street intervention team is a part of the public safety
system. A range of mental health treatment is
available through the agency's PATH project. Detoxification is provided. Project enhancements include a vision clinic,
providing free eye exams and eyeglasses, a full dental program,
outreach eligibility work, medication funds,
community case management, and a new HOPE (Social Security
Assistance) project. More than 100 volunteers
work in the project,
including physicians, dentists, optometrists,
opticians and hygienists from the community.
Linkages
Ties exist with the entire Eugene human service
network, including government, nonprofit, church,
and private entities. Health system linkages include hospitals,
the health department, specialists, labs, diagnostic
services, and professional societies (medical, dental, optometric,
and hygienist). The project works cooperatively
with local shelters, detox, public safety, and local hospitals.
WBHHC is a member of Lane County's mental health,
substance abuse, medical, and information/referral systems.
Coordination is arranged through the 100% Access
Project, Community Mental Health Consortium, Lane County Substance Abuse Coordinating Group, Eugene's
Community Response Team, and WBHHC's Interagency Management-Planned
Approaches to Community Treatment (IMPACT) Network. WBHHC is also a member of the City
of Eugene's Public Safety/911 system through
CAHOOTS, its mobile intervention team.
