Project Description

Care Alliance provides primary health care and comprehensive dental services at three clinic sites as well as at numerous outreach clinics, shelters, churches, and other locations where homeless people congregate. Care Alliance is also a Ryan White Title III grantee and a participant in the National Health Care Diabetes Collaborative.

Services

Services include primary care, dental care, HIV testing and tracking, short-term mental health and chemical dependency treatment and referral, and referrals for specialty care through community linkages. Through outreach services and primary care clinics in public housing and other locations, Care Alliance is able to provide a seamless system of service delivery across the life span continuum.

Linkages

Care Alliance works collaboratively with a number of health care and homeless service providers in the area, including AIDS Task Force, Catholic Charities, City Mission, Community Assessment and Treatment Services, EDEN, Lesbian/Gay Community Service Center, Lutheran Ministry, Mental Health Services, Neighborhood Family Practice, NEON, Office of Homeless Services, Proyectoluz, St. Malachi Center, Salvation Army, Transitional Housing, University Hospitals of Cleveland, Metro Health Systems, West Side Catholic Center, Y-Haven, and Departments of Health and Senior and Adult Services. Care Alliance is a member of the Ryan White Title I and Title II Consortiums, as well as several other community organizations and councils.

Project Description

The Cincinnati Health Network contracts with eight community agencies and/or organizations to provide comprehensive health care services to persons who are homeless in the greater Cincinnati, Northern Kentucky area.

Services

The Cincinnati Health Network contracts with the following: City of Cincinnati Board of Health (operates a Mobile Health Unit traveling a fixed weekly route to shelters and street locations to deliver medical services), University Family Physicians (provides physician services for the Mobile Unit), Health Point Family Health Care (operates a 24 hour/week free-standing homeless clinic in Covington, Kentucky), Center for Chemical Addictions Treatment (provides medically monitored detoxification and treatment services), Health Resource Center (provides mental health services including assessments, treatment, therapy and medications; medical care is also available to patients at this location), Greater Cincinnati Oral Health Council (operates McMicken Dental Clinic, which provides a full range of oral health care at a state-of-the-art homeless dental facility), Drop-Inn Center (operates a residential drug/alcohol treatment facility for homeless men in the largest adult shelter in the city), FreeStore/Food Bank (provides case management, representative payee services, referrals and housing assistance to homeless individuals and families), and Center for Respite Care (a 15 bed facility that provides 24 hour medical respite services to homeless individuals).

Linkages

Approximately 40 human service agencies were involved in the planning of Cincinnati Health Network, representing major providers of medical care, mental health/substance abuse treatment, public assistance, housing, emergency food, shelter, and other social services. The Greater Cincinnati Coalition for the Homeless, an informal network of homeless service providers, is actively involved in the project. Supportive and referral services are provided by community health services and the Ryan White Title III program.

Project Description

Columbus Health Care for the Homeless is a subsidiary of Columbus Neighborhood Health Center (CNHC). CNHC comprises five health centers that have been providing family oriented, affordable, comprehensive primary care for more than 25 years. The HCH philosophy is to ensure that persons who are homeless have access to comprehensive primary health care and mental health and substance abuse services. This is done through a multidisciplinary team at clinical care sites, homeless gathering sites, and shelters.

Services

Columbus HCH services include adult medicine, pediatrics, immunization, OB/GYN, family planning, podiatry, social service, laboratory, dental, vision, WIC, transportation, pharmaceutical, health education, nutrition counseling, outreach, case management, assistance with referrals, linkage to community resources, Columbus Health Department programs, substance abuse, mental health, safe shelter, permanent housing, entitlement, and job services. Substance abuse services include outreach to single men and women who are nonabstinent. These services are provided through an affiliation with The Open Shelter, permanent supportive housing linkage, care coordination, case management, and other stabilizing services that are provided to single men in early recovery.

Linkages

Crisis intervention and mental health services are provided through a relationship with NETCARE ACCESS. Three other community mental health agencies provide counseling and case management. Other relationships include the Columbus Health Department programs for alcohol and drug abuse treatment, sexual health, HIV/AIDS, breast and cervical cancer prevention, lead poisoning prevention, prenatal services, and home visitation. Additional linkages are established with Southeast Mental Health and Veteran Administration.

Project Description

Neighborhood Health Association (NHA) administers the Senior Services Center and operates six community health centers, a pediatrics center, two school-based sites, and the Mildred Bayer Clinic for the Homeless.

Services

Mildred Bayer Clinic delivers primary medical, dental, and vision care. Services provided on-site–or within the NHA network–include podiatry, pediatric care, immunizations, OB/GYN, social services, laboratory, WIC, transportation, health education, nutrition, counseling, case management, outreach, pharmacy, eyeglasses, dentures repairs, and referral. Nurses are stationed at three area shelters.

Project Description

Samaritan Homeless Clinic is a department of Good Samaritan Hospital. It is co-located within Dayton's only daytime homeless shelter and in walking distance of the emergency night shelters. All integrated, comprehensive, multidisciplinary services are housed on-site. Off-site, the clinic runs an apartment respite (recuperative) care program in the independent living section of an adult care center.

Services

Clinic services include primary care, psychiatry, gynocology, podiatry, dental care, optometry, mental health and substance abuse counseling, life skills education, outreach, transportation, and case management. Laboratory, radiology, OB, referrals, access to emergency room and inpatient care are provided by the hospital and other off-site referrals. Residential substance abuse treatment and mental health case management services are accessed through Crisis Care, which is a Good Samaritan Hospital program.

Linkages

The Samaritan Homeless Clinic is active in Shelter Providers Committee and Emergency Housing Coalition, a group of some 28 public, private, and not-for-profit agencies that have a major involvement with persons who are homeless and collaborate to maximize services and reduce duplicate efforts. The clinic also participates in the Continuum of Care process, Ten Year Plan to End Homelessness, and in several U.S. Department of Housing and Urban Development (HUD) Supportive Housing Programs. Patients graduating from the clinic are transitioned to neighborhood health centers with a sliding scale fee. The Combined Health District provides HIV education/testing, TB care/follow-up, and childhood immunizations. The AIDS Resource Center Ohio provides medical care, case management, and housing.
