Project Description

HAWC (Health Access Washoe County) Outreach Medical Clinic is a satellite facility of the HAWC Community Health Center, located in the heart of the homeless area on Motel Row.

Services

The outreach facility provides primary care, mental health counseling, medical screening, drug/alcohol screening and counseling, and referrals. Two chiropractors volunteer their services. The clinic is backed up by the main HAWC facility's team of physicians and dentists.

Linkages

HAWC Outreach Clinic has linkages with Restart, Nevada Mental Health Institute, Department of Veteran's Affairs, and NASAC.

Project Description

Nevada Health Centers' (NVHC) HCH Program provides primary health care, case management, outreach, and referral services through its Las Vegas Outreach Clinic (LVOC), Downtown Outreach Clinic (DOC), and a mobile outreach van. Services are provided for people who are homeless and at-risk of homelessness in the greater Las Vegas/Clark County area. Nevada Health Centers operates 23 clinics throughout Nevada, including a mobile mammography van and 3 mobile dental vans.

Services

Comprehensive primary medical services are provided through two shelter-based clinics. LVOC is staffed by a fulltime physician, medical assistant, and receptionist, and DOC is staffed by a PA, medical assistant, and receptionist. Both clinics have a lab and on-site dispensary. The HCH Outreach Team is based at the LVOC clinic. The clinics offer a full range of medical services and referrals to one of NVHC's three CHC clinic sites in Las Vegas, all of which offer pediatric and adult medicine, prenatal and postpartum care, STD and HIV/AIDS screening/referrals, TB testing, senior care, referral for housing, SA/MH counseling, legal assistance, job services, government assistance, and more. OB/GYN care is offered through the NVHC OB/GYN clinic in North Las Vegas. Referrals are arranged for dental care, optometry, and medical specialties. A team of full-time eligibility workers assist patients with referrals to County, State, and Federal agencies.

Linkages

Both HCH clinics and the outreach team work closely with shelters, local homeless service providers, and county and city agencies, including the county hospital, to provide a continuum of care for patients. NVHC has close working relationships with local volunteer dentists and optometrists, Las Vegas Metro Police Department, and other outreach organizations. NVHC's HCH program is part of a team with Mojave Mental Health under a HRSA/SAMHSA collaborative grant.
