Project Description
AtlantiCare Health Services Health Care for
the Homeless project provides primary health
care and substance abuse, mental health, and
outreach services within a case management framework
for persons who are homeless in Atlantic County.
Services
AtlantiCare Health Services is located in the
Atlantic City Rescue Mission. The project provides
primary care and integrative behavioral health
services through an expansive multidisciplinary
team. In addition, an ambulatory detoxification
program is offered with intensive outpatient
treatment.
Linkages
AtlantiCare has linkages with 25 social service
agencies to provide services within the community.

Project Description
Henry J. Austin Health Center operates at
four locations in the greater Trenton area.
Its Health Care for the Homeless program is
located in a new, state-of-the-art facility
across the street from a shelter for homeless
individuals and a new transitional housing program
for families. The project employs two full-time
adult medicine/GYN providers, a part-time pediatrician,
behavioral health care, and social work services
at its Ewing Street location. These services
complement the full lifecycle of primary care
offered at Austin's North Warren Street location
and its satellite sites at Chambers Street in
Trenton and in Hamilton Township.
Services
Henry J. Austin Health Center at Ewing Street
serves men, women, and children in a full spectrum
of primary care. Services also include behavioral
health, social work, podiatry, infectious disease
treatment, HIV and Hepatitis C testing and treatment,
outreach, referrals, and health promotions/disease
prevention. Austin Center participates in the
BPHC Diabetes Collaborative. Staff provide mobile
health screenings throughout the city, including
a local shelter, soup kitchen, and several transitional
housing programs. Austin Center at Ewing Street
anticipates expanding its dental services at
this new facility.
Linkages
Ewing Health Center staff work closely with
Community Options shelter, Escher Street SRO
Housing, Greater Trenton Behavioral Healthcare,
Homefront services to homeless families with
children, Rescue Mission, Salvation Army, Trenton
Area Soup Kitchen (TASK), and other community-based
agencies.

Project Description
The Homeless Program uses a multidisciplinary,
integrated model, and client-centered, team
approach. Six culturally-competent and culturally
diverse staff members serve the homeless population
in Hudson County and beyond.
Services
The Homeless Program provides primary care,
mental health services, referral for HIV-antibody
testing, outpatient substance abuse counseling,
and screening services for diabetes, hypertension,
and tuberculosis. A pharmacy is on-site. Referrals
are made for gynecological, dental, mammogram,
and podiatry services. The program uses a variety
of outreach methods and community connections
to identify and serve homeless individuals,
particularly those with co-occurring substance
abuse and mental health issues. Staff members
include physicians, a nurse practitioner, a
licensed social worker/therapist, counselors,
and a van driver.
Linkages
Linkages for dental, mammogram, podiatric,
and HIV services are provided. The Homeless
Program also has linkages with all of the shelters
in the community.

Project Description
Newark Homeless Health Care was originally
funded in 1985 by the Robert Wood Johnson Foundation.
The project is administered by the City of Newark,
Department of Health and Human Services, under
the Division of Medical Care Services. This
linkage to the city's health department provides
an array of additional services for individuals
experiencing homelessness in Newark.
Services
Newark Homeless Health Care provides health
care, social services, and substance abuse/mental
health counseling services at various shelters
within the city of Newark. On-site medical services
are provided by five medical teams in five shelters
and one multi-service center. A Special Care
Clinic has been established at the Health Department
for patients who are homeless. Services provided
on-site include medical history/screening, blood
work, immunizations, prescription assistance,
service referrals, and an array of other primary
care services. Additional funding has allowed
the project to expand medical services to individuals
with HIV/AIDS. In addition to the core services
of primary care, Newark Homeless Health Care
provides HIV/AIDS patients with support services,
including case management, transportation, and
group support. Newark has several outreach initiatives,
including an adult immunization team and a social
services component at the Newark Liberty International
Airport, the busiest airport in the New York
metropolitan area, where intervention, case
management, shelter assistance, transportation,
and medical service referrals are provided to
homeless persons residing at the airport complex.
Linkages
Various preventive, diagnostic, and therapeutic
services are obtained through a direct referral
system generated on-site. Services include x-rays
for diagnosis of chest diseases, substance abuse
treatment, and follow-up.

Project Description
Project H.O.P.E. uses a multifaceted approach,
including mobile health outreach, street outreach,
and primary care clinics, to reach people who
are homeless. These components are intended
to fill the gaps in the continuum of services
which have been identified through a network
of homeless service providers in the city and
through Project H.O.P.E.'s 9 years of experience.
Project H.O.P.E. strives to provide the highest
quality of client-centered care to men, women,
and children.
Services
Project H.O.P.E. operates two clinics. Bergen
Lanning Health Center is Project H.O.P.E.'s
"medical home" for the homeless. A
satellite clinic is conducted weekly at the
Volunteers of America Aletha Wright Shelter/Transitional
Living Program. This clinic provides physicals,
infectious disease screening, housing referrals,
mental health counseling, case management, and
social services. Transportation assistance is
available. Case management and follow-up continue
for 1 year after permanent housing is obtained.
Project H.O.P.E.'s mobile health van travels
to Camden City sites providing assessments,
primary care services, screenings, emergency
care, TB testing, health care education, drug/alcohol
counseling, and employment and housing counseling.
Staff conduct intakes, assess needs, case manage,
counsel, and refer clients. The Street Outreach
component travels to 32 sites, distributing
meals and care packages. All Project H.O.P.E.
staff participate in street outreach.
Linkages
Project H.O.P.E. partners with several other
agencies in Camden City to provide a continuum
of care and smooth transition between services.
These agencies include, but are not limited
to, Camden Area Health Education, The Bridge,
Cathedral Kitchen, Camden County Board of Social
Services, Camden County Department of Health
and Human Services, Department of Veterans Affairs,
Food Bank of South Jersey, Homelessness Prevention
Program, Interfaith Homeless Outreach Council,
Leavenhouse, My Brothers Keeper, Neighborhood
Center, New Visions Community Services, South
Jersey Behavioral Health Resources, and Volunteers
of America - Delaware Valley.
