Project Description

HCH Manchester offers primary medical care, behavioral health care, substance abuse counseling, case management, health education, and nursing outreach to men, women , children and teens who are homeless in Manchester. Through its long-standing partnerships, HCH Manchester extends its reach beyond Manchester, by facilitating clinical assistance and access to care for people who are homeless throughout the entire State.

Services

HCH Manchester offers primary care clinics daily,on site at two shelter locations: at New Horizons for New Hampshire, the State's largest emergency shelter for single men and women, and at Families In Transition, a multi-site, statewide transitional housing program for families, and adults. HCH Manchester conducts Nursing Street Outreach 4 days per week in partnership with the local PATH clinician and Homeless Runaway Youth team. HCH Street outreach extends beyond the streets to riverbanks, woods, smaller shelters, and drop-in centers, as well as to those families living doubled-up in substandard housing within diverse ethnic and refugee communities. HCH-Health Education sessions are conducted weekly, on site, at various shelters on a rotating basis. Classes are offered to patients. shelter guests and shelter staff alike. Generous State funding and recent partnership with The Mental Health Center of Greater Manchester, have enabled twice weekly Behavioral Health clinic sessions that are fully integrated within the HCH Manchester system of primary care, offering psychiatry on site at New Horizons shelter, for the very first time.

Linkages

For twenty years, HCH Manchester has maintained long-standing partnerships and extensive linkages with virtually all health and human service agencies, and academic institutions throughout the State of New Hampshire and the greater Manchester community.

Project Description

Two mobile health teams travel within a 100 square mile area, visiting shelters, soup kitchens, camp grounds, and other isolated areas frequented by individuals who are homeless. The team consists of a project coordinator, nurse, nurse practitioner, alcohol/substance abuse counselor, care coordinator, and mental health counselor.

Services

Services include primary medical care, medication, substance abuse counseling and referrals, laboratory and diagnostic testing, oral health treatment and referral, immunizations, medical/social care coordination, health education, eligibility screening, and tracking and follow-up through an electronic medical records system.

Linkages

Linkages include health and social service agencies (AIDS Response-Seacoast, Alliance for Community Health, Stafford Network), shelters (Cross Roads House, My Friend's Place, Rochester Overflow Shelter, A Safe Place, New Generation), community health centers (Avis Goodwin CHC, Lamprey Health Care), hospitals (Exeter, Portsmouth Regional, Frisbie, Wentworth-Douglass), mental health centers (Seacoast Mental Health/Community Partners), a network of volunteer physicians and dentists administered by SeaCare Health Services, U.S. Department of Housing and Urban Development (HUD) Continuum of Care (United Way of the Greater Seacoast, City of Portsmouth), and outreach (Rockingham Community Action, Strafford Community Action).

Project Description

Harbor Care Clinic is a new access point grantee made possible through funding from the ARRA. On June 30, 2009, Harbor Care Clinic began providing primary and preventive care services to people experiencing homelessness in the greater Nashua area. Harbor Care Clinic operates primarily from one fixed location with two fully equipped exam rooms. Outreach and health education are also conducted at shelters and other areas frequented by individuals who are homeless.

Services

Services include primary medical care, medication, substance abuse couseling and referrals, mental health counseling and referrals, laboratory and diagnostic testing, medical/social care coordination, and eligibility screening. Harbor Care Clinic utilizes electronic medical record and practice management systems.

Linkages

Harbor Care Clinic has established linkages with health and social service agencies throughout the greater Nashua area including Lamprey Health Care, a 330(e) grantee also located in Nashua.
