Project Description
The mission of Homeless Health Services (HHS)
is to provide comprehensive primary health care
to people
experiencing homelessness in the Fargo-Moorhead
area.
Services
HHS is a program of Family Health Care Center
in Fargo. The project maintains a primary care
clinic in the downtown area at St. Mark's Lutheran
Church. Clinic hours are Monday through Friday,
8:30 am to 12:30 pm. Services include management
of acute and chronic health problems, preventive
care, and patient education. Patients receive
case management services, including referrals
for mental health and substance abuse services.
Outreach is provided by staff at area shelters
and other community sites serving people who
are experiencing homelessness.
Linkages
Homeless Health Services has linkages with
Family Health Care, Fargo Cass Public Health,
Merit Care Medical Center, Southeast Human Service
Center, Lakeland Mental Health, area shelters,
and transitional housing sites.
