Project Description
The Goshen Medical Center (GMC) – Eastpointe
homeless project provides comprehensive health
care and mental health services to the homeless
and migrant population. To accomplish these
goals, GMC-Eastpointe collaborates with the
County Health Department, Social Services, Housing
Authority, and vocational rehabilitation. Since
their three sites, GMC-Eastpointe, GMC-Salvation
Army, GMC-Goldsboro, are located near a military
base, veterans and their family members also
use the extensive services provided.
Services
Comprehensive health care and mental health
services are available 5 days a week. Outreach
services are provided 2 to 3 nights. Dental
services are provided by a contracted dentist
at the office and by a charitable organization,
“Baptist Men’s Mobile Dental Unit,”
in the migrant camps. The sliding fee scale
serves as a tremendous benefit for the patients
and makes health and dental care reasonably
affordable for this population. Referrals for
specialty care are accomplished through subcontractors
or other partnering agencies.
Linkages
GMC-Eastpointe has linkages with Salvation
Army, Wayne County Health Department, Wayne
County Social Services, Wayne County Housing
Authority, Lighthouse, and Fordham House.

Project Description
Lincoln Community Health Center operates a
Health Care for the Homeless project on the
campus of the Urban Ministries Shelter and Community
Kitchen in downtown Durham. Medical and social
work services are provided to men, women, and
children who are residing at any of Durham's
shelters, recovery centers, domestic violence
facilities, and to those who are living on the
streets or in their cars.
Services
Lincoln's HCH clinic provides comprehensive
primary care services and urgent care services.
Services are provided weekdays by a family nurse
practitioner and during evening clinics (3-4
times a week) by a physician assistant or volunteer
community physicians. A clinical social worker
is available during most of the clinics for
counseling and case management services and
for a weekly coping skills group. In addition,
a podiatrist provides services once a week.
At the main Lincoln clinic, specialty and dental
services are available.
Linkages
The project has strong linkages with other
area health and mental health agencies. Lincoln
Community Health Center, in collaboration with
the Durham County Health Department, operates
an Early Intervention Clinic for HIV-positive
and AIDS patients. Family planning, prenatal
care, and tuberculosis screening and treatment
services are also offered by the County Health
Department located across the street from the
Health Care for the Homeless Clinic. Local Management
Entity (LME), The Durham Center, which is charged
with managing mental health, developmental disabilities
and substance abuse services in Durham County,
sustains strong linkages with the Health Care
for the Homeless Project and the Projects for
Assistance in Transition from Homelessness (PATH)
project, which provides transitional housing.
Substance abuse counseling continues to be provided
by staff at the project’s shelter site.

Project Description
New Hanover Community Health Center’s
service delivery model includes a free-standing
facility designed to provide primary health
care and limited dental services. Primary health
care services include physical examinations,
diagnosis and treatment for chronic diseases,
treatment for upper respiratory infections and
skin diseases, screening for communicable diseases,
and podiatry screening. The clinic site is adjacent
to a homeless shelter. The clinic is staffed
by one mid-level medical provider, a part-time
dentist, a social worker, community outreach
worker, and clinic coordinator.
Services
The program provides primary health care and
dental services. Mental health and substance
abuse services are available through referral.
Other services on-site include Minority Infant
Mortality Reduction Program, Breast and Cervical
Cancer Control Project, HIV/AIDS testing and
counseling, pharmacy, and Department of Social
Service Eligibility determination. Services
also include the physical health appraisal,
acute care, laboratory, health screening, health
education and promotion, STD prevention and
treatment, HIV and Hepatitis B prevention and
treatment, adolescent sexuality and development,
pregnancy prevention and counseling, immunizations,
mental health assessments and counseling, case
management, substance abuse (including alcohol
and tobacco) prevention and treatment, management
of chronic physical problems such as asthma,
diabetes, and seizure disorders, nutrition and
physical fitness counseling, dental screening,
and referrals.

Linkages include Tri-County Homeless Interagency
Council, Domestic Violence Shelter and Services,
and Southeastern Center for Mental Health, Development
Disabilities and Substance Abuse Services.

Project Description
Horizon Health Center is a free-standing medical practice serving men, women and children experiencing homelessness in Raleigh and Wake County. It is one of eight practices operated by Wake Health Services, a multi-site community health center organization. Primary health services are also offered at South Wilmington Street Center, the largest men's shelter in the county.
Services
Comprehensive primary care services are provided by a .5 FTE Family Practice Physician, a 1.0 FTE Family Nurse Practicioner,a .1 FTE Board Certified Internal Medicine/Pediatrics/Infectious Disease Physician, a Registered Nurse and a Licensed Clinical Addiction Specialist. Services include acute care, management of chronic health conditions including HIV, referrals to medical specialists, post-hospital and surgical follow up, access to medications, labs, diabetes education and substance abuse services.
Linkages
Horizon Health Center founded and actively participates in a county-wide diabetes collaborative addressing education, access to medications, diabetes supplies and specialty shoes. Horizon provides HIV adherence counseling through a partnership with Alliance of AIDS Services Carolina. Horizon collaborates with area shelters to identify medically at risk individuals who need shelter during extreme weather as well as medical respite coordination. Horizon works with pharmaceutical company programs to meet patients' medication needs. Horizon coordinates care with wrap-around mental health agencies and is an active participant with the local coalition to end homelessness.
