Project Description

Community Health Partners (CHP) is a community health center organization with two medical sites, a dental site, and a full literacy/GED and ESL program. In 2006, an HCH program began to serve seasonal farm workers in Park County, a sparsely populated, frontier county in south-central Montana.

Services

Approximately 502 seasonal workers and their families are targeted to receive primary health care services, including preventive health care, injury prevention, substance abuse treatment, tobacco use cessation, and mental health services. CHP's HCH project employs a medical provider (0.6 FTE), case management staff, health educators, lay outreach workers (a rural Promotora model), as well as other support staff, and has expanded on-site access to literacy services, GED opportunities, job retraining (on-site and through partnerships), and English as a Second Language services.

Linkages

Linkages include Bozeman Deaconess Hospital, Livingston Memorial Hospital, The Mental Health Center, and Southwest Chemical Dependency Center.

Project Description

Yellowstone City-County Health Department is the primary grantee for four HCH programs located in Billings, Helena, Missoula, and Butte. Services are provided at shelter-based clinics and community health centers.

Services

Services for the HCH program include primary health care, mental health care, substance abuse treatment, dental services, case management, and outreach services.

Linkages

Linkages include community agencies which provide various services, including substance abuse treatment, respite care, residential services, a domestic violence shelter, and vocational education.
