Project Description

Coastal Family Health Center, an FQHC, provides primary health - care to all individuals in Hancock, Harrison and Jackson Counties. Coastal has seven clinics located across the three counties and anyone coping with homelessness may access healthcare during the hours of 8 a.m. to 5 p.m. However, in the Biloxi Clinic a dedicated homeless clinic is offered form 1 p.m. to 8 p.m. on Tuesdays and can be accessed by phone contact or walk-ins. There is also a mobile unit in Pass Christian that serves the homeless during regular clinic hours.

Services

Community outreach is an ongoing and integral part of the homeless health clinic. The clinic offers medical, dental, optometry,social services and substance abuse counseling, including individual and group sessions. The clinics have a point-of-service lab and on-site pharmacy.

Linkages

The center works collaboratively with numerous local health and social service agencies that provide resources and/or services needed by patients. Among the most used are the Departments of Human Services within Hancock, Harrison and Jackson Counties, Gulf Coast Mental Health Center, Seashore Mission, Gulf Coast Homeless Shelter, Biloxi Medical Center, all regional housing, Gulf Coast Community Action Agency, Catholic Social Services, Salvation Army, South Mississippi AIDS Task Force, and Homeless Continuum of Care. Since Hurricane Katrina, linkages have included Hands on USA, Volunteers of America, Episcopal-Lutheran Disaster Response, Americore, NACHC, Extreme Makeover, South Carolina Primary Healthcare, Children's Health Fund, Islamic Nation, Academy of Physician Assistants, Rotary, Direct Relief Fund, Merek Inc., and Yakima Valley WA Migrant Health Center.

Project Description

Jackson-Hinds Comprehensive Health Center's Health Care for the Homeless project provides services from one central site. Patients, however, can be served at any Jackson-Hinds clinic locations.

Services

The project accepts referrals from the Gateway Rescue Mission, Salvation Army, and several organizations that provide services to the homeless. The project includes a 0.6 FTE mid-level provider, a part-time MD, an LPN, a receptionist/medical assistant, and a social worker/director. The after-hours clinic is staffed by the Jackson-Hinds Comprehensive Health Center. The project provides primary health care, limited emergency health care, medications, immunizations, social services, mental health counseling and referral, substance abuse counseling/education and treatment by referral, health education, referral for inpatient services, outreach, some transportation, and eligibility assistance. Dental, optometry, laboratory, and other referrals are provided from JHCHC. The project contracts with Southeast Mississippi Rural Health Initiative as the grantee for the Ryan White Title III HIV/AIDS Program. This program provides HIV/AIDS counseling, testing, treatment, medications, and case management services.

The project has linkages with the following organizations: MS State Hospital, Harbor House, Alcohol Services, New Life for Women, Friendship Connection, New Dimensions, Genesis House, Victory Manor, Country Oaks Recovery Lodge, Friends of Alcoholics, Falcon Hill, Lighthouse Mission Ministries, Daylight Ministries, Common Bond VA Homeless Program, New Life A & D, Miracle House, Metro Counseling Services, and Personal Care Homes (recovery); Day Break, Gateway Rescue Mission, Salvation Army, Battered Women's Shelter, Billy Brumfield House, Matt's House, Sims’s House, Southern Christian Services for Children and Youth/PALS Program, and Wingard House (shelters); Hinds County Restitution Center, Federal Restitution Center, and Bannum Place (restitution); Snubbs Homeless Program/MS State Hospital, Hinds Behavioral Health, and Region 8 Mental Health Center (mental health); and Building Bridges, Hinds County Health Department, and University of Mississippi Medical Center (HIV/AIDS). In addition, a podiatrist accepts referrals. The project has rapport with social services agencies, educational programs, shelters, and food programs. The project director serves on coalitions and organizations that advocate for individuals who are homeless. The director is a member of social services networking organizations that work toward eradicating poverty, empowering clients, and promoting social justice.
