Project Description

Grace Hill Neighborhood Health Centers has been providing care to underserved communities since 1906. Since 1987, Grace Hill has administered the HCH program.

Services

Services include primary medical and dental care, OB/GYN, pediatrics, optometry, case management, WIC, pharmacy, and transportation. Grace Hill contracts various agencies for primary care, case management, and substance abuse/detox services. Grace Hill also provides respite care and has a mobile dental van.

Linkages

Grace Hill has linkages with nearly 30 shelters and several health and specialty care providers, including Washington University Medical School, Barnes/Jewish Hospital, and several schools of nursing and social work.

Project Description

Buchanan County has nearly 86,000 residents, with almost 74,000 residing in the city of St. Joseph. Buchanan County social services agencies and school documented nearly 1,200 homeless persons in the past year, a combination of approximately 150 chronic homeless and 1,050 temporarily homeless.

Project Description

Swope Health Services’ Health Care for the Homeless Program, located in Swope Health Services, a community health center and behavioral health center, provides primary health care and support services – outreach, case management, outpatient drug and alcohol services, counseling, medication, and dental for individuals who are homeless. U.S. Department of Housing and Urban Development (HUD) supportive service grants enable the program to provide prescription medication and collaboration with a behavioral health program to serve the chronic mentally ill. A recent SAMHSA grant award provides permanent housing with extensive support services to the chronic homeless through a housing first model.

Services

The HCH project is located within Swope Health Services and on-site primary care is provided through the following clinics: adult medicine, OB-GYN, pediatrics, dental, vision, and behavioral health. on-site auxiliary services include radiology, laboratory, pharmacy, and optical shop. Homeless outreach provides health and support services at shelters, soup kitchens, drop-in centers, treatment facilities, city jail, on the streets, homeless camps, and other sites where homeless persons congregate.

Through a Homeless Services Coalition, the program is linked to all community service providers.
