Services
The Mobile Health Team, comprising a full-time
nurse practitioner and two full-time RNs, conducts
outreach and refers patients to Downriver Community
Services, New Haven Medical Center, People's
Clinic for Better Health, and Neighbors Caring
for Neighbors for primary care and substance
abuse services. In addition, clinics have been
established in MATTS shelter and McRest Shelter.

Community Behavioral Health, Downriver Community
Substance Abuse Services, Community Mental Health,

Project Description
The Family Health Center Health Care for the
Homeless program was established in 1987 to
ensure access to primary health care and substance
abuse services through integrated systems of
care for people who are homeless . The HCH program
operates on the premises of the Ministry with
Community Center which houses the HCH clinic,
a drop-in center, an emergency overnight shelter,
a soup kitchen, and a food and clothing bank.
Services
The Family Health Center HCH Program provides
basic health, diagnostic, preventive, acute
or urgent care, and pharmaceutical services.
Primary care services are provided by a physician
and mid-level providers. HCH also conducts intensive
outreach, case management, housing, income,
and transportation linkage activities, as well
as referrals and funding for radiology, laboratory,
and dental services. HCH assesses mental health
and substance abuse issues and provides treatment
and/or referral when necessary.
Linkages
HCH has referral arrangements with Borgess
Medical Center, Bronson Methodist Hospital,
a number of specialty care providers in the
area, and Central Assessment Screening Services
for substance abuse counseling. Other linkages
include surrounding shelters and the Health
Disparities Collaborative sponsored by HRSA.

Project Description Family Health Center of Battle Creek (FHC), in southwestern lower Michigan in the cities of Battle Creek and Albion, has been delivering primary health care for Calhoun County residents since 1986; designated a CHC in 1992; HCH in 2007. FHC cares for approximately 20% of the County's population, 50% of the County's Medicaid population and 30% of the County's uninsured population.
Services
FHC employs a staff of 192 including board-certified physicians, dentists, nurse practitioners, physician assistants, social worker, holistic health coordinator, behavioral health specialist, substance abuse counselor and an audiologist. Support staff includes dental hygienists, nurses, medical and dental assistants, translators/interpreters, receptionists, schedulers, patient advocates, accounts representatives, patient information assistants, housekeeping, maintenance and administrative personnel.
Services provided include Family Practice, OB/GYN and Women's Health, Internal Medicine, Pediatric Medicine, Preventive Medicine, Ear-Nose-and-Throat, Complementary and Alternative Medicine, General Dentistry, social work, holistic health counseling, behavioral health and substance abuse counseling, health education, translation and deaf interpretation and a therapy pet. Pharmacy and lab services are on-site in Battle Creek; lab and x-ray services are on-site in Albion (provided by Oaklawn Hospital). FHC participates in a 340(B) Drug Pricing Program with the Federal Government and a Senior Millage Dental Program with Calhoun County. FHC also participates as a partner in the County Health Plan. The Center is a United Way of Greater Battle Creek agency as one of the community's safety net programs.

FHC has linkages with every provider of human services in Calhoun County as well as organizations, groups and agencies in Calhoun County concerned about human services and access to those services. These would include, to name only a very few: Battle Creek Health System, Oaklawn Hospital of Marshall, Calhoun County Department of Human Services, Summit Pointe (local Community Mental Health agency), Regional Health Alliance, City of Battle Creek, Calhoun County/Greater Battle Creek Homeless Coalition, The Coordinating Council, Albion Health and Wellness Action Team, Albion Health Care Alliance, Calhoun County health Department, Nursing Clinic of Battle Creek, Fountain Clinic of Marshall, shelters, food bank, fraternal and social organizations and churches.

Project Description
Through a mobile health team, Hamilton Community
Health Network (HCHN) provides comprehensive
primary health care to persons who are homeless
in Genesee County at seven clinic sites in missions,
shelters, and other multi-service agencies.
Services
An outreach coordinator, mid-level provider,
caseworker, and a medical assistant staff the
HCHN homeless program. Services include family
planning, prescription medication, laboratory
testing, preventive dental services, transportation,
OB program, TB testing, substance abuse, and
mental health referrals. A Family Independence
Agency eligibility worker, stationed at the
main clinic, assists clients in applying for
benefits and entitlements. In addition to providing
medical services, the mobile health team places
strong emphasis on providing health education
on the non-medical factors that influence homelessness
in the community. This approach is an attempt
to treat the whole person both physically and
mentally as part of the case management process.

Partnerships and linkages include Shelter
of Flint, REACH, Community Mental Health, Salvation
Army, Transition House, Odyssey House, Inner
City Outreach, and Carriage Town Mission.

Project Description
Sparrow Community Health Center is one of nine
stand-alone centers within the community health
center network of the Ingham County Health Department.
It is the designated site for the provision
of health care services to homeless individuals.
Outreach and service delivery is provided at
homeless shelters by a mobile health team.
Services
Primary care medical services comprise the
bulk of services rendered. Augmenting these
services are participation in Title X (Family
Planning), Title XV (Breast and Cervical Cancer
Control Program), Vaccines for Children, on-site
substance abuse treatment, and smoking cessation
counseling. STI/HIV screening and treatment
is available as well as enabling services by
a social worker, nutritionist, and other support
staff. Other services, such as dental, are provided
through referral within Ingham County’s
Community Health Center Network.
Linkages
Networking with a local neighborhood center
has yielded improved access to health care.
Involvement with Michigan State University (MSU)
Extension service offers a referral source to
assist with chronic disease management. Attending
physicians from MSU provide volunteer after
hours medical services and limited access to
specialty care on-site.

Project Description
Saint Mary's Health Care provides services
to individuals who are homeless in Grand Rapids'
Downtown Heartside area and the Southwest Corridor.
Services
Heartside Clinic provides primary health care
and support services to persons who are homeless,
mainly those living in shelters or on the street
in the downtown area of Grand Rapids. Clinica
Santa Maria provides primary health care, OB/GYN,
podiatry, dental, dietician, maternal and infant
support services, and WIC to persons who are
homeless, many of whom are transient or living
with relatives and are Spanish speaking. Both
sites offer "open access," offering
both pre-scheduled and same-day appointments.
Linkages
Emergency care, laboratory, specialty clinic
services, and prescription medicine are provided
at a nearby hospital campus. The HCH program
has established liaisons with Kent County Community
Mental Health, Kent County Health Department,
Family Independence Agency, local missions and
shelters, Salvation Army, an array of substance
abuse facilities, Catholic Social Services,
and Dwelling Place of Grand Rapids, which is
the major provider of SROs offering temporary
housing.

Project Description
Detroit Health Care for the Homeless (DHCH)
maintains five outreach sites and a stationary
clinic in Detroit. The stationary clinic operates
five days per week.
Services
Direct services include prevention, medical,
nursing, dental, social work, and pharmacy.
Mental health and substance abuse services are
provided on-site by DHCH staff in addition to
referrals to community agencies and treatment
facilities.
Linkages
Subspecialty, hospital, emergency, laboratory,
and radiology services are provided by community
resources. Social service problems are addressed
by clinic social/outreach workers and a network
of referrals in the community. Substance abuse
treatment is available to individuals with alcohol/drug
abuse problems who are homeless through community
agreements and the Detroit Health Department
Bureau of Substance Abuse. Psychiatric treatment
is available through an agreement with Managed
Care Group of Detroit and Wayne County (formally
Detroit Central City Community Mental Health).
DHCH also has linkages with New Center Mental
Health, Detroit Health Department, and Visiting
Nurses Association. St. John Health (System)
provides specialty referral services and hospitalization.
